盐酸利多卡因对猪灌注模型中肾盂压力的影响

Effect of endoluminal lignocaine irrigation on renal pelvic pressure in a porcine model of intrapelvic perfusion

摘要 目的探讨1%的盐酸利多卡因在不同灌注流速下对肾盂压力的影响。方法建立动物模型并随机将猪5只(肾脏10个)分为A组(灌注利多卡因)和B组(灌注生理盐水),其中一肾分至A组,那么另一肾自动分至B组。测量基础肾盂压力5 min后,灌注实验分为3个阶段,阶段1:两组均以8 ml/min的速率灌注生理盐水5 min。阶段2:A组以8 ml/min的速率灌注1%的盐酸利多卡因5 min,而B组以生理盐水替代。阶段3:两组均依次以8 ml/min、16 ml/min、24 ml/min的速率各灌注生理盐水5 min。实验全程均监测肾盂压力值、心率和呼吸频率。结果阶段1时A、B两组的基础肾盂压力分别为(12±4)和(13±6)cm H_2O(1 cm H_2O=0.098 k Pa)(P>0.05)。两组均灌注8ml/min生理盐水时的肾盂压力分别为(51±12)和(54±16)cm H_2O(P>0.05)。阶段2:两组的肾盂压力分别为(47±9)和(52±15)cm H_2O(P>0.05)。阶段3:两组均依次以8 ml/min、16 ml/min、24 ml/min的速率灌注生理盐水的肾盂压力分别为(43±10)和(50±17)cm H_2O(P>0.05),(49±15)和(61±17)cm H_2O(P<0.05),(54±16)和(65±14)cm H_2O(P<0.05)。整个实验过程中两组的心率和呼吸频率差异均无统计学意义。结论 1%的盐酸利多卡因可降低与灌流速率相关的肾盂压力。 Obiective To investigate the effects on the pressure-flow relation of renal pelvic pressure (RPP) with intraluminal administration of 1% lignocaine in pigs. Methods The animal models ofintrapelvic perfusion in pigs were constructed. This experiment was randomized to two groups. Baseline: A 5 minutes period for baseline RPP measurement. Period 1: A 5 minutes period of irrigation using saline in both groups. Period 2: A 5 minutes period of irrigation with randomization for irrigating 1% lignocaine (group A) or saline (group B). Then after perfusion was done on the other side as a control in each pig. The Irrigation rate period 1 and 2 was standardized to 8 ml/min. Period 3: A 15 minutes period of irrigation using saline in both groups. During the first 5 minutes, the irrigation rate was 8 ml/min. And then the irrigation rate was changed to 16 ml/min in the second 5 minutes. In the third 5 minutes, the irrigation rate was 24 ml/min. The renal pelvic pressure, heart rate and respiratory rate were recorded. Result The mean baseline pelvic pressures in A and B group were (12±4) and (13-6) cmH2O (P〉0.05). The mean pelvic pressures in period 1 and period 2 were (51±12) and (54±16) cmH20, (47±9) and (52±15) cmH20 (P〉0.05). The mean pelvic pressures in period 3 at the irrigation rate of 8 mL/min, 16 ml/min and 24 ml/min were (43 ± 10) and (50±17) cmH2O (P〉0.05), (49±15) and (61±17) cmHzO (P〈0.05), (54±16) and (65±14) cmH2O (P〈0.05), respectively. The mean heart rate and respiratory rate did not change significantly.Conclusion We conclude that 1% lidocaine can decrease pressure-flowrelation.
