摘要
目的分析影像学检查骨膜三角、针状骨膜反应征象与骨病变良恶性的关系。方法收集行影像学检查并经病理检查证实有骨膜反应的病人897例,根据病理结果分成恶性病变组85例、良性病变组812例,比较两组骨膜三角、针状骨膜反应征象的出现情况。结果恶性病变组影像学表现为骨膜三角征象43例(50.6%),良性病变组6例(0.7%),两组比较差异有显著性(χ~2=360.6,P<0.01)。恶性病变组影像学表现为针状骨膜反应30例(35.3%),良性病变组4例(0.5%),两组比较差异有显著性(χ~2=246.1,P<0.01)。结论骨膜三角、针状骨膜反应征象在良恶性骨病变中均可出现,但多见于恶性骨肿瘤,少数情况下也可见于快速进展的多种良性骨病变。
Objective To analyze the association of Codman triangle and acicular periosteal reactions in imaging examination with benign and malignant bone diseases. Methods This study consisted of 897 patients who had periosteal reactions found in imaging examination and confirmed by pathology.The patients were divided into malignant-lesion group(n=85)and benign-lesion group(n=812)according to pathological results.The appearance of Codman triangle and acicular periosteal reactions between the two groups was compared. Results Imaging features of Codman triangle and acicular periosteal reactions were noted as follows:in the malignant group,Codman triangle was found in 43cases(50.6%),and in the benign group,it was six cases(0.7%),the difference was significant between the two groups(χ^2=360.6,P〈0.01).In the malignant group,acicular periosteal reactions were observed in 30cases(35.3%),and in the benign group,it was four(0.5%),the difference was significant(χ^2=246.1,P〈0.01). Conclusion Codman triangle and acicular periosteal reactions can be found both in malignant and benign bone lesions,more common in malignant tumors,but in a few cases,which can also be seen in the rapid progression of a variety of benign bone lesions.
