摘要
[目的]观察宣肺行水汤联合西药治疗慢性肺心病急性加重期合并心衰疗效。[方法]使用随机平行对照方法,将78例住院患者按抛硬币法简单随机分为两组。对照组39例溶栓:尿激酶,2万U/kg+9%生理盐水100mL,1次/d,静滴;抗凝:肝素,负荷量为2000~3000U/h,连续静滴;抗休克:多巴胺20mg+200mL 5%葡萄糖,1次/d,静滴。治疗组39例宣肺行水汤(山茱萸、丹参、沙参、黄芪各15g,麦冬、桑白皮、黄芩各10g,紫苏子、瓜蒌各8g;法半夏、甘草各6g;气阴两虚加党参15g;肺肾虚亏加山药15g),1剂/d,水煎200mL,早晚口服,100mL/次;西药治疗同对照组。连续治疗14d为1疗程。观测临床症状、消失时间(咳嗽、咳痰、气喘、气短)、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效25例,有效12例,无效2例,总有效率94.87%。对照组显效12例,有效15例,无效12例,总有效率69.23%。治疗组疗效优于对照组(P<0.01)。时间指标治疗组改善优于对照组(P<0.01)。[结论]宣肺行水汤联合西药治疗慢性肺心病急性加重期合并心衰疗效满意,无严重不良反应,值得推广。
[Objective] To observe the water line Xuanfei soup combined with Western medicine with acute exacerbation of chronic pulmonary heart disease with heart failure treatment. [Methods] Random parallelcontrol method, 78 cases of hospitalized patients by a coin toss method is simple randomized into two groups. The control group 39 cases thrombolytic： urokinase 20,000 U/kg＋9% 100mL saline, l/d, intravenous infusion; anticoagulant： heparin, the load capacity of 2000-3000U/h, continuous intravenous infusion; anti-shock ： dopamine 20mg＋200mL 5% glucose, 1 times/d, intravenous infusion. Treatment of 39 patients Xuanfei line of water soup （Shanzhuyu, Danshen, Shashen, Huangqi each 15g, Maidong, Sangbiapi, Huangqin 10g, Zisuzi, Gualou 8g; Fabanxia,Gancao 6g; gas Yin deficiency Dangshen 15g; lung kidney deficiency Shanyao 15g）, 1 Ji/d, decoction 200mL, sooner or later, oral, 100 mL/times; western medicine with the control group. 14d for a course of continuous treatment. Observing clinical symptoms （cough, sputum, wheezing, shortness of breath） disappearance time of adverse reactions. One course of treatment, to determine efficacy. [Results] Treatment group 25 cases markedly effective in 12 cases, 2 cases, the total efficiency of 94.87%. Control group, 12 cases markedly effective in 15 cases, 12 cases, the total efficiency of 69.23%. Treatment group than the control group （P〈0.01）. Time metrics to improve the treatment group than the control group （P〈0.01）. [Conclusion] Xuanfei line of water decoction combined with western medicine with acute exacerbation of chronic pulmonary heart disease with heart failure results were satisfactory, no serious adverse reactions, should be promoted.
