摘要
目的探讨格雷夫斯病(Graves's disease,GD)患者血浆中神经肽Y(neuropeptide Y,NPY)和降钙素基因相关肽(calcitonin gene-related peptide,CGRP)水平及其与甲状腺激素之间的关系,为GD的诊治寻找新的诊疗指标。方法收集GD患者80例,分为3组:(1)治疗前组:未接受治疗的患者;(2)缓解组:甲硫咪唑片治疗后缓解的患者;(3)治愈组:甲硫咪唑片治疗后达到治愈标准的患者,采用放射免疫分析方法检测各组中CGRP、NPY水平变化,并与其FT3、FT4、TSH、TRAb进行相关性分析。结果血浆中CGRP、NPY水平治疗前组(51.1±11.1ng/L、825.1±41.9ng/L)和缓解组(106.3±12.8ng/L、630.8±84.5ng/L)明显不同于治愈组(144.3±12.3ng/L、138.1±24.4ng/L),将3组的数据进行两两比较,差异有统计学意义(P<0.05);在治疗后组中FT3、FT4、TSH恢复到正常时,而CGRP、NPY还处于异常水平;CGRP、NPY分别与组间对应的FT3、FT4、TRAb、TSH进行线性回归分析。结果显示3组FT3、FT4、TRAb、TSH与CGRP和NPY均相关,同时CGRP、TSH与GD严重程度呈现负相关(R=-0.933,-0.944,P=0.000,0.000),NPY、FT3、FT4、TRAb则表现为正相关(R=0.926,0.943,0.930,0.943,P=0.000,0.000,0.000,0.000)。结论血浆中CGRP、NPY与GD密切相关,CGRP在GD中显著降低,而NPY显著增高,有望成为GD防治的诊疗指标。
Objective The aim of this study was to investigate the relationship among the levels of CGRP,NPY,and thyroid hormones in plasma of patients with Graves’ s disease( GD). Methods 80 definite GD patients were designated as prior treatment group when they had not been treated with methimazole and as post-treatment group after treatment,at last as cured group after cure. The levels of CGRP and NPY in all groups were detected by radioimmunoassay,and the relationship with FT3,FT4,TRAb,and TSH was analyzed. Results The contents of CGRP in the prior treatment,post-treatment,and cured group were( 51. 1 ± 11. 1,106. 3 ± 12. 8,144. 3 ±12. 3) ng/L,and the contents of NPY were( 825. 1 ± 41. 9,630. 8 ± 84. 5,138. 1 ± 24. 4) ng/L respectively.There were significant differences( P 〈 0. 05) among the three groups. The levels of CGRP and NPY were returned to normal more slowly than FT3,FT4 and TSH in the post-treatment group. The linear regression analysis results showed that FT3,FT4,TRAb and TSH should be related to CGRP and NPY. CGRP and TSH were negatively correlated with the severity of GD( R =-0. 933,-0. 944,P = 0. 000,0. 000),and NPY,FT3,FT4,TRAb were positively correlated with GD( R = 0. 926,0. 943,0. 930,0. 943,P = 0. 000,0. 000,0. 000,0. 000). Conclusion CGRP and NPY could be associated with GD and reflect the changes of thyroid hormone regularly. It is suggested that CGRP and NPY would be considered as diagnostic indexes for GD.
