摘要
目的探讨老年糖尿病肾病患者的营养状况的影响因素及其饮食护理干预的效果。方法将2015年9月—2016年10月的60例糖尿病肾病伴随营养不良的患者,随机分组为观察组和对照组,对照组采取常规护理措施,观察组基于常规护理的基础上,采用专门的饮食护理进行干预,比较两组治疗后的营养指标水平、钙磷水平、胃泌素。同时选取同期糖尿病肾病非营养不良患者60例作为非营养不良组,分析老年糖尿病肾病患者营养状况的影响因素。结果 (1)老年糖尿病肾病患者营养状况的影响因素包括:膳食知识、膳食信念、膳食行为、Ca、2 h PG、BUN、BUA、24 h蛋白尿。(2)与对照组对比,观察组患者治疗后各项营养指标更优,TSF、AMC、TP、ALB水平和钙磷水平更高,ALP水平更低,数据比较差异具有统计学意义,P<0.05。结论对老年糖尿病肾病患者进行营养评估和饮食护理干预,能够有效地改善患者的身体营养状况,为透析治疗的顺利进行奠定良好的身体条件。
Objective To investigate the influencing factors and the effect of diet nursing intervention for nutritional status in elderly patients with diabetic nephropathy. Methods 60 cases of diabetic nephropathy with malnutrition of patients from September 2015 to October 2016, were randomly divided into observation group and control group, the control group received the routine nursing, the observation group on the basis of routine nursing, the special diet nursing intervention, two groups were compared after treatment of nutritional indexes, and gastrin levels of calcium and phosphorus. At the same time, 60 patients with diabetic nephropathy were selected as non malnutrition group. The influencing factors of nutritional status in elderly patients with diabetic nephropathy were analyzed. Results(1) The influencing factors of nutritional status in elderly patients with diabetic nephropathy include: dietary knowledge, dietary beliefs, dietary behavior, Ca, 2 h PG, BUN, BUA, 24 h proteinuria.(2) and the control group, the observation group after treatment were significantly better in the nutritional index, TSF, AMC, TP, ALB levels of calcium and phosphorus levels were significantly higher, ALP level was significantly lower, with statistically significant difference data, P〈0.05. Conclusion Nutritional assessment and diet nursing intervention for elderly patients with diabetic nephropathy can effectively improve the nutritional status of patients, and lay a good physical condition for the smooth treatment of dialysis.
