摘要
目的探讨经直肠超声(TRUS)行前列腺穿刺活检术诊断前列腺癌诱发尿路感染的相关因素及护理措施。方法收集2013年5月—2016年5月进行TRUS前列腺穿刺活检的102例患者的临床资料进行回顾性分析,将患者分为感染组与未感染组。通过多因素Logistic回归分析术后合并尿路感染的危险因素,并分析开始治疗时间与治疗3 d后尿白细胞计数关系。结果 102例患者中15例出现尿路感染症状;经尿培养与细菌鉴定,14株致病菌中,革兰阴性致病菌占11例,其中大肠杆菌9例。多因素分析显示,患者有尿路感染史、合并前列腺感染、年龄>70岁、术前未使用抗生素、留置尿道管时间>4 d与术后诱发尿路感染有显著相关性(P<0.05),均为危险因素。15例术后并发尿路感染患者给予抗生素治疗效果随开始治疗时间延长而呈明显下降趋势。结论合并尿路感染的致病菌主要是大肠杆菌,而有尿路感染史、合并前列腺感染、年龄>70岁、术前未使用抗生素、留置尿道管时间>4 d的患者则有较高风险发生尿路感染。临床护理应对这些危险因素加以重视。
Objective To investigate the potential risk factors of urinary tract infection （UTI） caused by transrectal ultrasound （TRUS） guided prostate biopsy, and provide related nursing measures. Methods A retrospective analysis based on 102 prostate cancer patients undergoing transrectal ultrasound （TRUS） guided prostate biopsy was carried out. Patients were divided into the infected and the uninfected group. Multivariable logistic regression analysis was performed to investigate the potential factors of UTI. The relationship between time of using antibiotic and treat-ment effect （level of urinary-leukocyte） was illustrated by the curve. Results In 102 patients who underwent biopsy, 15 patients（14. 71% ） with symptoms of UTI. There were 14 strains of pathogenic bacteria detected by UC and bacteria identification, in which gram-negative bacteria accounting for 11 cases, including 9 cases of e. coli. Multivariate analysis showed that history of urinary tract infection, merging, infections of the prostate, age 〉70, preoperative use of antibiot-ics, indwelling catheter time 〉 4 days were correlated with postoperative UTI （P 〈 0. 05）. The treatment effect on 15 patients appeared a downward trend with time prolonging. Conclusion In the process of UTI after biopsy, escherichia coli. plays an important role in pathogenic bacteria. And people have a history of UTI, infections of the prostate, age 〉 70 years old,didn’ t use of antibiotics previously, with indwelling urethral tube time 〉 4 days have higher risk of suffering UTI. It is required to carry out antibiotic therapy to control the condition.
