摘要
目的:研究分析血清N端脑钠肽原(NT-proBNP)水平对婴幼儿肺炎心力衰竭的诊断价值。方法:选取2015年1月-2016年12月本院收治的85例支气管肺炎患儿为研究对象,按照中华医学会儿科学分会心血管学组制定的《小儿心力衰竭的诊断与治疗》中小儿心力衰竭改良Ross评分标准将患儿分为无心衰组43例(评分0~2分),轻度心衰组22例(评分3~6分),中-重度心衰组20例(评分7~12分),并选取同期在本院体检健康的40例健康儿童为对照组。检查记录各组研究对象NT-proBNP水平、肌钙蛋白I(cTnI)水平、肌酸激酶同工酶(CKMB)水平、心脏左室射血分数(LEVF)、左室缩短分数(LVFS)并进行比较。结果:四组研究对象血清中NT-proBNP、cTnI、CKMB水平比较,差异均有统计学意义(P<0.05),中-重度心衰组NT-proBNP、cTnI、CKMB水平均高于其他各组(P<0.05);而轻度心衰组NT-proBNP、cTnI、CKMB水平均高于无心衰组和对照组(P<0.05);四组研究对象LVEF、LVFS比较差异均有统计学意义(P<0.05),中-重度心衰组LVEF、LVFS均低于其他各组(P<0.05);轻度心衰组LVEF、LVFS均低于无心衰组和对照组(P<0.05)。相关性分析表明,NT-proBNP水平与改良Ross评分呈正相关,而与LEVF、LVFS呈负相关(P<0.05)。结论:血清中NT-proBNP、cTnI、CKMB水平与肺炎合并心力衰竭的严重程度密切相关,且与左心室的功能呈负相关,而NT-proBNP是诊断婴幼儿肺炎合并心力衰竭的特异性指标,可以准确地反映心力衰竭的严重程度。
Objective: To study the diagnostic value of serum N terminal brain natriuretic peptide ( NT-proBNP ) levels in infants with pneumonia and heart failure.Method: In our hospital, 85 cases of bronchial pneumonia from January 2015 to December 2016 were selected as the research objects, according to the subspecialty cardiology group to develop the "children of heart failure diagnosis and treatment of pediatric heart failure" in the modified Ross standard for evaluation of children were divided into 43 cases of non heart failure group ( 0-2 scores ), 22 cases of mild heart failure group ( 3-6 scores ), 20 cases of moderate to severe heart failure group ( 7-12 scores ), and 40 healthy children in our hospital physical examination were selected as the control group, inspection records of each research object, troponin NT-proBNP level cardiac troponin I ( cTnI ), creatine kinase ( CKMB ) level, left ventricular heart ejection fraction ( LEVF ), left ventricular fractional shortening ( LVFS ) .Result: NT-proBNP, cTnI, CKMB were compared in four groups of subjects's serum level difference ( P〈0.05 ), moderate to severe beart failure group NT-proBNP, cTnI and CKMB were higher than those of other groups ( P〈0.05 ) .NT-proBNP, cTnI and mild heart failure group and CKMB were higher than those of non heart failure group and the the control group ( P〈0.05 ) .Four groups of subjects LVEF LVFS, the difference in severe heart failure group LVEF and LVFS than those of the other groups ( P〈0.05 ) .Mild heart failure group LVEF and LVFS than those of mild heart failure without heart failure group and the control group ( P〈0.05 ) .Correlation analysis showed that NT-proBNP level was positively correlated with the modified Ross score, but negatively correlated with LEVF and LVFS ( P〈0.05 ) .Conclusion: The conclusion is closely related to the severity of serum NT-proBNP, cTnI, CKMB and pneumonia combined with heart failure, and negatively correlated with le
