摘要
目的:观察经皮穴位电刺激对体外受精-胚胎移植(in vitro fertilization-embryo transfer IVF-ET)反复移植失败(repeated implantation failure RIF)患者子宫内膜血流的影响及其作用机制。方法:将64例RIF患者随机分为治疗组及对照组,每组各32例。对照组仅给予常规用药,治疗组在对照组的基础上于月经干净后开始运用经皮穴位电刺激治疗,于排卵后停止刺激。检测子宫内膜螺旋动脉血流搏动指数(PI)、阻力指数(RI)和子宫内膜血流分型情况。结果:治疗组子宫内膜的血流分型治疗前后有明显改善,其子宫内膜螺旋动脉血流PI、RI值均较治疗前低,差异均有统计学意义(P<0.05)。治疗后治疗组与对照组比较,子宫内膜血流分型改善较多,其子宫内膜螺旋动脉血流PI、RI值均较对照组低,差异均有统计学意义(P<0.05)。结论:经皮穴位电刺激八髎穴可以改善IVF-ET反复移植失败患者子宫内膜血流情况,从而提高子宫内膜的容受性,利于胚胎着床。
Objective： To observe the influence of transcutaneous electrical acupoint stimulation on endometrial blood flow of patients with in vitro fertilization-embryo（ IVF-ET） transfer repeated implantation failure（ RIF） and its action mechanism. Methods： Sixty-four patients with RIF were randomly divided into the control group and the treatment group,with 32 cases in each group. The ones in the control group were given routine treatment,while the ones in the treatment group were added transcutaneous electrical acupoint stimulation after menstruation based on the treatment of the control group,. and stopped the stimulation after ovulation. Then the indexes of blood flow pulsatility index（ PI）,resistance index（ RI） and endometrial blood flow were measured.Results： In the treatment group,there was a significant improvement in blood flow after the treatment. The PI and RI values of the spiral arteries in the endometrium were lower than those before the treatment,and the differences were statistically significant（ P〈0. 05）. After the treatment,the treatment group＇s endometrial blood flow classification improved more than that of the control group,and its uterine spiral artery blood flow PI,RI values were all lower than those of the control group,and all the differences were statistically significant（ P〈0. 05）. Conclusion： Transcutaneous electrical acupoint stimulation on Baliao points an improve endometrial blood flow of patients with in vitro fertilization-embryo（ IVF-ET） transfer repeated implantation failure（ RIF）,so as to improve the endometrial receptivity for smooth embryo implantation.
