摘要
目的观察微波与咪喹莫特乳膏外用联合光动力疗法治疗鲍温样丘疹病（BP）的临床效果。方法选取2008年10月至2015年11月河北省唐山市协和医院收治的BP患者47例。完全随机分为观察组（21例）与对照组（26例）。对照组采用微波与咪喹莫特乳膏外用治疗，观察组在对照组基础上联合光动力疗法治疗。治疗1个月后观察比较2组有效率及痊愈率，随访观察治疗后3、6、12个月的复发率。比较2组微波治疗次数及咪喹莫特乳膏应用时间及不良反应发生情况。结果观察组痊愈率和有效率高于对照组、治疗后3个月复发率低于对照组，差异均有统计学意义[90．5％（19／21）比19．2％（5／26）、95．2％（20／21）比65．4％（17／26）、4．8％（1／21）比26．9％（7／26）]（均P〈0．05）；治疗后6、12个月2组复发率差异无统计学意义（P〉0．05）。观察组微波治疗次数少于、咪喹莫特乳膏应用时间短于对照组，差异均有统计学意义[（2．0±1．1）次比（4．2±1．9）次、（2．2±0．7）个月比（4．7±1．6）个月]（均P〈0．01）。2组均有轻度不良反应出现，未影响继续治疗及局部功能。结论微波与咪喹莫特乳膏外用联合光动力疗法可明显提高鲍温样丘疹病的疗效并减少复发。
Objective To observe the clinical effect of microwave therapy and external use of imiquimod cream combined with photodynamic therapy on Bowenoid papulosis. Methods A total of 47 patients with Bowenoid papulosis from October 2008 to November 2015 in Tangshan Union Medical College Hospital of Hehei Province were randomly divided into observation group （21 cases） and control group （26 cases）. The control group was treated with microwave therapy and imiquimod cream for external use; the observation group was added photodynamic therapy. Effective rate and curative rate were analyzed after 1 month of treatment; recurrence rate was analyzed 3, 6, 12 months after treatment. Times of microwave therapy, imiqnimod cream use time and adverse reactions were recorded. Results Curative rate and effective rate in observation group were significantly higher and 3 month recurrence rate was significantly lower than those in control group E90. 5% （19/21） vs 19.2% （5/26）, 95.2%（20/21） vs 65.4% （17/26）, 4. 8% （1/21） vs 26.9% （7/26）] （P 〈0.05）; 6, 12 month recurrence rates had no significant differences between groups （ P 〉 0. 05 ）. Times of microwave therapy and imiquimod cream use time in observation group were significantly lower than those in control group [ （ 2. 0 ± 1.1 ） times vs （4. 2 ± 1.9） times, （ 2. 2 ± 0. 7 ） months vs （4. 7 ± 1.6 ） months ] （ P 〈 0.01 ）. Only mild adverse reactions were observed in both groups. Conclusion Microwave therapy and imiquimod cream combined with photodynamic therapy treating Bowenoid papulosis can improve clinical effect and reduce recurrence.
