摘要
目的探讨患者周围物品与医务人员污染对ICU多药耐药菌(MDRO)医院感染的影响,并采取针对性防控措施,为降低ICU MDRO医院感染率提供理论依据。方法采用前瞻性调查方法,对2014年5-9月ICU患者周围物品、医护人员工作衣及其鼻腔、手部、咽部MDRO污染情况进行监测,并采取消毒隔离措施进行干预,对其医院感染情况进行统计分析。结果 ICU患者周围物品、医护人员工作衣、鼻腔、手部及咽部MDRO总检出率为3.71%,其中患者周围物品MDRO检出率最高,显著高于医护人员鼻腔、手部及咽部的MDRO检出率,差异均有统计学意义(P<0.05);工作衣MDRO检出率仅次于患者周围物品MDRO检出率,差异无统计学意义;实施干预后,送检标本MDRO总检出率呈逐月下降趋势;ICU患者MDRO医院感染率有所下降,与干预前比较差异有统计学意义(P<0.05)。结论医院应高度重视患者周围物品及医护人员工作衣污染MDRO对患者的不良影响,尤其要认真做好ICU患者周围物品中床单和被套及医护人员工作衣的妥善处置等细节管理工作,提高感染管理质量,保障患者安全。
OBJECTIVE To investigate the influence of patient related materials and the medical staff infection on nosocomial infections caused by multidrug-resistant bacteria（MDRO）in ICU,and take appropriate measures,so as to provide theoretical basis to effectively reduce the ICU MDRO nosocomial infection rates.METHODS From May to Sept.2014 the MDRO infection of patient related materials in ICU,the work cloths of medical staff and their organs such as noise,throat and hands were monitored by prospective investigation.At the same time,sterilization and isolation measures were applied to intervene the MDRO infection.Then,the nosocomial infection situations were statistically analyzed.RESULTS The total MDRO detection rates of patient related materials in ICU,the work cloths of medical staff and their organs was about 3.71%.The detection rate of patient related materials was the highest,which was obviously high than that of the noise,hands and throat of medical staff,and the difference was significant（P〈0.05）.The detection rate of the work cloths of medical staff（6.40%）was just lower than that of patient related materials,and there was no difference in statistics.After the intervention,the total MDRO detection rate decreased month by month,the infection rate of patients in ICU also decreased obviously,and the difference was significant compared with that before intervention（P〈0.05）.CONCLUSION Hospital should pay more attention to the adverse effects of MDRO of the patient related materials and the work cloths of medical staff.In particular,it should do a good job of detail management of proper disposal of peripheral items of sheets and quilt and work clothes of medical staff of ICU patients,to improve the quality of infection management and guarantee the health of the patients.
