摘要
[目的]通过三维有限元分析骨折断端微动及应力情况,为LCP(locking compression plates,LCP)"动力化"临床相关应用提供理论依据。[方法]建立骨折近端锁定孔锁定螺钉,远端全锁定螺钉固定(A组);骨折近端锁定孔普通螺钉,远端全锁定螺钉固定(B组);骨折近端加压孔普通螺钉,远端全锁定螺钉(C组)固定胫骨中下段横形骨折的有限元模型,各组分别予以1/6、2/6、3/6、4/6、5/6及1倍体重应力值进行轴向压缩实验,评价各组骨折断端轴向微动距离、胫骨及内固定最大应力以及断端应力值等指标。[结果]相同轴向应力载荷下,胫骨最大应力:A<B<C,内固定最大应力:A>B>C;骨折端轴向微动位移:A<B<C,两两比较差异具有统计学意义(P<0.05);骨折断端应力:A<B<C,两两比较差异具有统计学意义(P<0.05)。[结论]B、C两组动力固定方式在体重应力范围内可为骨折断端提供有效的轴向微动;且较A组固定能明显减少内固定的应力,增加骨折断端应力。
[Objective] To investigate the micromovement and stress in fracture site with finite element analysis, and to provide theoretical basis for clinical application. [Method] Finite element modeling software was used to build three internal fixation models for treating tibial transverse fracture. The fracture was fixed with all locking screws in Group A. In group B, common screws was fixed in locking holes at proximal and locking screws was fixed at distal position. While in GroupC, com- mon screws was fixed in compression hole at proximal and locking screws was fixed at distal position. Compressive stress tests were applied in each group with 1/6, 2/6, 3/6, 4/6, 5/6 and 1 times the weight valued. The maximum stress of tibia and in- ternal fixation, micromovement , stress in fracture site were observed and compared among three groups. [Result] Under same axial stress load, the max stress of tibial was A〈B〈C, the max stress of internal fixation was A〉B〉C, the micromove- ment in fracture site was A〈B〈C, the stress in fracture site was A〈B〈C, respectively. The paired comparisons showed signifi- cant difference among those groups (P〈0.05) . [Conclusion] Under certain axial stress load, two kinds (B, C) of in- ternal fixations can generate effective micromovement, reduce stress in the fixed device and increase stress in fracture site.
