摘要
目的通过调查分析湖北省内首例人感染H5N6禽流感患者的实验室检查结果,了解新发疾病H5N6禽流感的实验室检查特点,为H5N6禽流感提供更好的诊治依据。方法采集确诊患者的血液、体液标本,检测各项实验室检查指标。结果本省首例感染H5N6患者白细胞与中性粒细胞总数升高而红细胞减少;淋巴细胞呈现波动趋势;丙氨酸氨基转移酶(ALT)、天冬氨酸转氨酶(AST)在病毒核酸转阴过程中由明显增高后逐步恢复正常;肾功能、血脂糖等指标无明显异常;动脉血二氧化碳分压(pCO_2)持续增高,氧饱和度由低值转为正常;降钙素原(PCT)含量正常;辅助性T细胞(CD3^+CD4^+)降低,抑制性T细胞(CD3^+CD8^+)升高,CD4^+/CD8^+倒置;心肌肌钙蛋白I(cTnI)与N-末端脑利钠肽(NT-proBNP)含量升高。结论 2016年全球第7例湖北省首例H5N6禽流感患者临床表现较重、病程较长,掌握H5N6禽流感实验室检查的特点可为其诊治提供有效的参考依据。
Objective By investigating and analyzing the laboratory results of the first human case of H5N6 avian influenza in Hubei,to understand the laboratory features of newly emerging disease H5N6 avian influenza,so as to provide effective evidence for the clinical treatment and diagnosis of H5N6 avian influenza. Methods Blood and body fluid samples were collected,and the laboratory indexes were monitored. Results The total amount of leucocytes and neutrophile granulocyte increased,but the number of red blood cells reduced,and the changes of lymphocyte showed a trend of fluctuations in the case. Moreover,the levels of serum alanine aminotransferase（ ALT） and aspartate aminotransferase（ AST） first significantly increased,and then gradually decreased to normal in the process of negative transformation of virus replication. There was no remarkable change of renal function indices,blood sugar and lipid levels. The partial pressure of carbon dioxide（ pCO2） continuous increased and oxygen saturation increased from low to normal value. Besides,the levels of serum procalcitonin（ PCT） were normal. Helper T cells（ CD3^＋CD4^＋） decreased and suppressor T cells（ CD3^＋CD8^＋） increased,therefore the ratio of CD4^＋,CD8^＋（ CD4^＋/CD8^＋） reversed. The value of cardiac troponin I（ cTnI） and N-terminal pro-brain natriuretic peptide NT-proBNP（ N-BNP） increased. Conclusion It presented a more severe clinical manifestation and a longer course in the world＇s first seventh cases of H5N6 avian influenza in Hubei. Mastering the characteristics of laboratory examination of H5N6 avian influenza can provide effective reference for its diagnosis and treatment.
