摘要
目的探讨厄贝沙坦治疗高血压性肾衰竭的临床疗效。方法选取盐亭县人民医院2014年10月—2016年5月收治的高血压性肾衰竭患者126例,按照随机数字表法将患者分为对照组与研究组,各63例。对照组予以硝苯地平及美托洛尔治疗,研究组在对照组基础上予以厄贝沙坦治疗。比较两组患者治疗前后收缩压、舒张压、血尿素氮、血肌酐及肾小球滤过率,观察不良反应发生情况。结果治疗前两组患者收缩压与舒张压比较,差异无统计学意义(P>0.05);治疗后研究组收缩压与舒张压低于对照组(P<0.05);治疗后两组收缩压与舒张压水平低于治疗前(P<0.05)。治疗前两组患者血尿素氮、血肌酐及肾小球滤过率比较,差异无统计学意义(P>0.05);治疗后研究组患者血尿素氮、血肌酐水平低于对照组,肾小球滤过率高于对照组(P<0.05);治疗后两组患者血尿素氮、血肌酐水平低于治疗前,肾小球滤过率高于治疗前(P<0.05)。两组患者均未发生严重不良反应。结论厄贝沙坦治疗高血压性肾衰竭的临床疗效确切,可有效降低患者血压,改善肾功能,且不良反应少。
Objective To discuss the clinical effect of irbesartan in treating hypertensive renal failure. Methods A total of 126 cases of patients with hypertensive renal failure were selected from October 2014 to May 2016 in Yanting People＇ s Hospital,which were divided into control group and study group according to random number table,63 cases in each group. Control group was treated with nifedipine and metoprolol, study group was treated with irbesartan based on control group. The SBP,DBP,blood urea nitrogen,serum creatinine and GFR before and after treatment were compared between the two groups,and the incidence of adverse reactions were observed. Results Before treatment,no statistically significant differences of SBP or DBP was found between the two groups（ P〉0. 05）; after treatment,the study group of SBP,DBP were lower than control group（ P〈0. 05）,and the two groups of SBP,DBP were lower than prior treatment（ P〈0. 05）. Before treatment,no statistically significant differences of blood urea nitrogen,serum creatinine or GFR was found between the two groups（ P〉0. 05）; after treatment,the study group of blood urea nitrogen,serum creatinine were lower than control group,GFR was higher than control group（ P〈0. 05）,and the two groups of blood urea nitrogen,serum creatinine were lower than prior treatment,GFR was higher than prior treatment（ P〈0. 05）. There was no adverse reactions happened. Conclusion Irbesartan has an exactly clinical effect in treating hypertensive renal failure,it can effectively decrease the level of blood pressure and improve renal function,and with less advese reactions.
