摘要
目的探讨腹腔镜与传统开腹手术对结直肠癌患者应激反应和胰岛素抵抗的影响,以评估腹腔镜手术治疗结直肠癌的优势。方法选取2012年7月至2014年12月在本院进行手术治疗的80例结直肠癌患者作为研究对象,根据手术方式分为腹腔镜组和传统开腹组,对两组患者术前与术后不同时间点的血清应激反应和胰岛素抵抗指标进行检测和比较。结果与手术前比较,两组患者的CRP、MDA和ROS均在术后第1天达到高峰后开始回落,而SOD则与之相反。术后第1、3、7天,腹腔镜组的CRP、MDA和ROS均低于传统开腹组,而SOD则高于传统开腹组(P<0.05)。两组患者的FBG、FINS和HAMA-IR亦在术后第1天达到高峰后开始回落。术后第1、3、7天,腹腔镜组的FBG、FINS和HAMA-IR均低于传统开腹组(P<0.05)。结论接受腹腔镜手术治疗的结直肠癌患者术后的应激反应和胰岛素抵抗程度要小于传统开腹手术,更有利于患者术后康复。
Objective This paper was to investigate the effects of laparoscopic surgery and traditional open surgery on stress reaction and insulin resistance for colorectal cancer (CRC) patients, in order to evaluate the advantages of laparoscopic surgery for CRC. Methods Eighty CRC patients treated with surgery in our hospital from July 2011 to December 2012 were divided into laparoscopic surgery group (LS group, n= 42) and traditional open surgery group ( TOS group, n = 38 ) according to the surgical methods. The indexes of stress reaction and insulin resistance of the two groups were detected and compared before the surgery and at different times after the operation. Results Compared to the preoperative indexes, CRP, MDA, and ROS of both two groups reached the peak at the first day after surgery, and then began to fall, whereas SOD was varied in a contrary trend. On the 1st to 7th day after surgery, CRP, MDA and ROS of LS group were lower than those of TOS group, while SOD of LS group was higher than that of TOS group (P〈0. 05 ). Similarly, FBG, FINS and HAMA-IR of the two groups reached the peak at the first day after surgery, and then began to fall. On the 1st to 7th day after surgery, FBG, FINS, and HAMA-IR of IS group were lower than those of TOS group.Conclusious The degree of stress reaction and insulin resistance of CRC patients treated with laparoscopic surgery is smaller than those of CRC patients underwent traditional open surgery, so laparoscopic surgery is more benefit for the postoperative rehabilitation of CRC patients.
