摘要
目的:探讨冷凝集素对不同类型血液分析仪结果的影响及消除方法。方法:将存在红细胞冷凝集的标本于37℃水浴30分钟及置换血浆前后,分别于Sysmex XT 2000i五分类全自动血液分析仪和SysmexKX-21N三分类血液分析仪上检测,结果与镜下人工计数比较。结果:水浴前,两种血液分析仪均出现相应报警,白细胞和红细胞分布图明显异常。红细胞和HCT结果明显减低,MCV,MCH,MCHC和RDW结果均明显偏高,WBC和PLT结果表现不一。37℃水浴30分钟后,31例标本中29例血细胞分布图形和参数恢复正常。血浆置换后,31例标本除PLT外,各分布图形和参数均恢复正常,PLT值偏低。结论:红细胞冷凝集对两种不同原理的血液分析仪有不同的影响。两种消除方法中,37℃水浴30分钟可消除大部分的RBC凝集现象,但对冷凝集效价极高的标本,可选择血浆置换法,其PLT结果可参照37℃水浴30分钟后检测结果。
Objective: To investigate the effect of cold agglutinin on different types of blood analyzer and elimination methods. Methods: Specimens with cold agglutination were detected for blood cells before and after 37℃ water bathing for 30 minutes and plasma replacement, in Sysmex XT 2000i five automatic blood analyzer and SysmexKX-21N classification three classification blood analyzer. Results were compared with the manual microscope count results. Results: Both blood analyzers showed the corresponding alarm before water bathing, with white blood cells (WBC) and red blood cells (RBC) distribution obviously abnormal. The results of HCT and RBC decreased significantly, MCV, MCH, MCHC and RDW were significantly higher, and WBC and PLT showed inconsistent. After 37℃ water bathing for 30 minutes, the distribution of blood cell graphics and parameters of 29 specimens returned to normal in all 31 cases. After plasma exchange, the distribution pattern and parameters of all 31 samples were normal, except PLT, with PLT value lower. Conclusion: The erythrocyte cold agglutinin has different effects on the two blood analyzers with different principle. In both elimination methods, 37℃ water bathing for 30 minutes can eliminate most of the RBC agglutination phenomenon, but to the high titer of condensed plasma specimens, the plasma replacement can be chose, the results of PLT should refer to the test results of methods after 37℃ water bathing for 30 minutes.
