摘要
目的对比Onyx胶介入栓塞与伽玛刀治疗脑动静脉畸形的临床效果。方法选择2011年1月至2015年1月收治的脑动静脉畸形患者80例,按照随机数字法分为两组,各40例。观察组进行Onyx胶介入栓塞治疗,对照组实施伽马刀治疗。对所有入组者随访1年,比较两组病灶磁共振检查结果、随访期间内两组改良Rankin量表得分以及两组治疗过程中出现的并发症情况。结果观察组治疗后1年,其病灶完全消失的患者比率显著高于对照组(P<0.05),而消失一半以及病灶治疗后无变化甚至增大的患者比率显著低于对照组(P<0.05)。干预后3月和干预后1年,观察组改良Rankin量表得分均显著优于对照组(P<0.05)。观察组发生脑水肿、癫痫、血管破裂及血管痉挛总发生率显著少于对照组(P<0.05)。结论 Onyx胶介入栓塞治疗脑动静脉畸形,能有效缩小病灶大小,改善患者生活质量,且治疗不良反应少,优于伽玛刀,值得临床推广。
Objective To investigate the treatment effect of Onyx glue embolization and gamma knife in the treatoent of cerebral art^ venous malformation. Methods 80 cases of intracranial arteriovenous malformations patients were selected from January 2011 to January 2015. They were divided into two groups, according to the random number method, 40 cases in each group. The observation group was treated with Onyx glue embolization, the control group was treated with gamma knife, two groups were followed up for 1 year. The MRI scores of the two groups were compared, and the scores of the modified Rankin scale in the two groups during the follow - up period and the complication occurred during the treatment were compared. Results After 1 year follow - u p , the ratio of lesions completely disappeared in the observation group was significantly higher that of the control group ( P 〈0. 05) , and the proportion of the patients with half lesion disappearance or without lesion change even with increase after treatment was significantly lower than that of the control group ( P 〈 0.05) . After 3 months and 1 year intervention, the score of the modified Rankin Scale in the observation group was significantly better than that of the control group ( P 〈 0. 05 ) . Brain edema, epilepsy, vascu-lar rupture and vasospasm overall incidence in the observation group were significantly less than those of the control group ( P〈0.05) . Conclu-sion Onyx glue embolization of intracranial arteriovenous malformation, can effectively reduce the size of the lesion, improve the life quality of pa-tients ,and reduce adverse drug reactions, it is worthy of promotion.
