摘要
目的探讨发热待查(FUO)患者的病因构成、诊断方法及思路。方法方便选取2013年1月—2016年1月该院收住的80例门诊初诊为FUO的患者的最终诊断,观察患者病因构成、最终确诊方法及病因与最终确诊时间的关系。结果 80例FUO病因明确患者中,感染性疾病57例(细菌感染39例,病毒感染15例,支原体感染3例),占71.25%;非感染性疾病17例(肿瘤7例,免疫疾病10例),占21.25%。感染性疾病所占比重显著高于非感染性疾病,各类感染性疾病最终确诊时间也均短于对照组,差异有统计学意义(P<0.05)。最终确诊方法中,诊断性治疗与观察构成比最高,占40.00%,影像学和细菌学次之,分别占25.00%和16.25%。结论感染性疾病是发热待查的主要病因,诊断性治疗与观察、影像学检查、细菌学检查等诊断技术的联合应用对发热待查的早期诊断具有积极作用,值得临床推广使用。
Objective To discuss the pathogenesis, diagnosis method and thinking of patients with FUO. Methods 80 cases of FUO patients by first visit in the outpatient department in our hospital from January 2013 to January 2016 were Convenient selection, and the pathogenesis, final confirmation method and relationship between cause and final confirmation time of patients were observed. Results In the 80 FUO cases with definite cause, there were 57 cases with infectious disease including 39 cases with bacterial infection, 15 cases with virus infection and 3 cases with mycoplasma infection, accounting for 71.25%, and there were 17 cases with non-infection disease including 7 tumor cases and 10 cases with immune disease,accounting for 21.25%, and the ratio of infectious disease was obviously higher than that of the non-infectious disease, and final confirmation time of various infectious diseases were shorter than those in the control group, and the differences had statistical significance（P〈0.05）, and in the final confirmation method, the diagnosis treatment and observation ratio was the highest, accounting for 40.00%, and the imaging and bacteriology were the second, respectively accounting for 25.00% and16.25%. Conclusion The infectious disease is a major cause of FUO, and the combined application of diagnosis treatment and observation, imaging examination and bacterial examination has a positive effect on the early diagnosis of FUO, which is worth clinical promotion and application.
