摘要
目的探讨尼可地尔对高龄非ST段抬高型急性心肌梗死患者心功能和心肌损伤标记物I(cTnI)的影响。方法 2012年3月—2016年7月高龄非ST段抬高型急性心肌梗死患者96例,随机分为试验组(48例)和对照组(48例)。两组均给予积极的心肌梗死综合治疗,同期试验组在综合治疗基础上口服尼可地尔。两组患者均治疗3个月。观察两组治疗前和治疗后1、3、7、10 d血清肌钙蛋白I(cTnI)水平;观察两组治疗前后的脑钠肽(BNP)、左室射血分数(LVEF)、左室舒张末期内径(LVEDD)。结果试验组治疗3、7和10 d后血清中肌钙蛋白I(c Tc I)明显低于对照组(P<0.05)。治疗3个月后试验组LVEF较对照组明显升高(P<0.05);试验组LVEDD、BNP较对照组明显降低(P<0.05)。结论尼可地尔可以改善高龄非ST段抬高型急性心肌梗死患者的心功能,并起到保护心肌的作用。
Objective To evaluate influence of nicorandil on heart function and cTnI in advanced aged patients with non-ST segment elevation myocardial infarction （non-STEMI）. Methods Ninety-six patients in advanced aged patients with non-ST EMI in author＇s department of cardiology from March 2012 to July 2016 were randomized and divided into trial group（n=48） and control group（n=48）. Patients in two groups were given a comprehensive treatment of myocardial infarction;while the trial group was treated with oral nicorandil in addition of the comprehensive treatment. Patients in two groups were given three months of the treatment. Serum level of cardiac troponin I（cTnI） was detected in both groups before and 1,3,7,and 10 d after treatment. Brain natriuretic peptide （BNP）,left ventrieular ejection fraction （LVEF）,left ventricular end-diastolic dimension （LVEDD） were observed and compared before and after 3 months of the treatment between the two groups. Results The serum level of cTnI was significantly lower in the trial group than that in control group 3,7,and 10 d after treatment（P〈 0.05）. After 3 months of the treatment LVEF was significantly higher in the trial group than that in control group,while BNP and LVEDD were significantly lower in the trial group than those in control group （P〈0.05）. Conelution Nicorandil has definite myocardial protection and can improve heart failure in advanced aged patients with non-ST EMI.
