摘要 目的探讨不同类型肝泡型包虫病(AE)病灶边缘区CEUS参数特点。方法对45例患者48个肝AE的病灶行常规超声检查并分为浸润型、液化型和钙化型,后行CEUS检查,绘制时间-强度曲线,并分别记录不同类型肝AE病灶的中心区、边缘区和周围正常区的CEUS定量参数,包括峰值强度(pI)、达峰时间(Tp)、曲线下面积(AUC)、平均渡越时间(MTT),并进行统计学分析。结果浸润型、钙化型和液化型病灶边缘区pI、Tp和MTT差异有统计学意义(F=8.832、9.088、5.538,P均<0.01),AUC差异无统计学意义(F=2.957,P=0.062)。浸润型与液化型、钙化型与液化型pI、Tp和MTT比较差异有统计学意义(P均<0.01),浸润型与钙化型pI、Tp和MTT差异无统计学意义(P均>0.05)。病灶中心区和周围正常区的pI、Tp、AUC和MTT差异均无统计学意义(P均>0.05)。结论不同类型肝AE病灶的边缘区CEUS参数特点不同,浸润型和钙化型肝AE病灶的边缘区血液供应较液化型更丰富。 Objective To investigate the characteristics of CEUS quantitative parameters on the different types hepatic alveolar echinococcosis (AE) lesions. Methods Totally 48 hepatic AE lesions of 45 patients were checked out and three types (infiltrative, liquefaction and calcification type) were classified by conventional ultrasound (US), then the CEUS was performed. Time-intensity curve was obtained and CEUS quantitative parameters including peak intensity (pI), time to peak (Tp), area under the curve (AUC) and mean transit time (MTT) were recorded in the non-enhanced internal regions, marginal areas and normal regions around the lesions. The statistical analysis was performed. Results In the marginal areas of infiltrative, calcification and liquefaction type AE lesions, pI, Tp and MTT showed significant differences (F=8.832, 9.088, 5.538, all P〈0.01), while AUC had no tatistical difference (F=2.957, P=0.062). The pI, Tp and MTT between infiltrative type and liquefaction type, calcification type and liquefaction type all had statistical difference (all P〈0.01). The pI, Tp and MTT between infiltrative type and calcification type had no statistical difference (all P〉0.05). Meanwhile in the internal regions and normal regions around the lesions, pI, Tp, AUC and MTT had no significant difference (all P〉0.05) in different types. Conclusion In the marginal areas of lesions, there are different characteristics of CEUS parameters in different types of hepatic AE lesions. The blood supply of the marginal areas of infiltrative type and calcification type lesions is richer than that of the liquefaction type.
