摘要
目的探讨宫腔镜电切术治疗子宫内膜息肉的效果及其对妊娠结局的影响。方法以我院2012年1月至2014年11月间收治的40例子宫内膜息肉患者为研究对象,所有患者均进行宫腔镜电切术治疗,观察手术时间、术中出血量及并发症的发生情况,术后随访观察2年,比较患者手术前后的经量、经期及妊娠结局。结果本组40例子宫内膜息肉患者的手术成功率为100%,手术时间为(18.4±3.8)min,术中出血量为(30.3±8.4)mL,所有患者均未见术中及术后并发症的发生。宫腔镜电切术后患者的月经量评分为(68.5±10.4)分,月经期为(6.2±1.4)d,均显著低于术前的(205.8±72.6)分与(9.6±2.7)d;术后的足月分娩率(52.5%)明显高于术前(2.5%),不孕率(27.5%)明显低于术前(57.5%),自然流产率(5.0%)明显低于术前(12.5%),差异均有统计学意义(P<0.05)。结论宫腔镜电切术显著提高了术后足月分娩率,降低了不孕率及自然流产率,改善了患者的妊娠结局。
Objective To explore the curative effect and pregnancy outcome of hysteroscopic resection in treatment of endometrial polyps. Methods 40 cases of patients with endometrial polyps admitted to our hospital from January 2012 to November 2014 were selected as research objects. All cases received hysteroscopic resection. The operation time,intraoperative blood loss and complications were observed. After operation,all patients were followed-up for 2 years,and the menstrual period and amount of menses before and after operation were compared. Results The success rate of 40 patients was 100%,the operation time was（18.4 ± 3.8） min,the intraoperative blood loss was（30.3 ± 8.4） mL,and no intraoperative or postoperative complication was found in all cases. After operation,the score of amount of menses was（68.5 ± 10.4） points and the menstrual period was（6.2 ± 1.4） d,significantly lower than（205.8 ± 72.6） points and（9.6± 2.7） d before operation,with statistical differences（P〈0.05）. After operation,the term delivery rate was 52.5%,significantly higher than2.5% before operation,and the infertility rate and spontaneous abortion rate were 27.5% and 5.0% respectively,significantly lower than57.5% and 12.5% before operation,respectively（all P〈0.05）. Conclusions Hysteroscopic resection can significantly improve the postoperative term delivery rate,reduce the postoperative infertility rate and spontaneous abortion rate,and improve patients＇ pregnancy outcome.
