摘要
目的:探讨根管治疗术一次法治疗下颌第一磨牙牙髓炎、牙髓坏死、根尖周炎的临床效果。方法:选择2014年11月-2016年3月在笔者所在医院口腔科接受治疗的患有下颌第一磨牙牙髓炎、牙髓坏死、根尖周炎疾病的患者150例,共150颗患牙,在疾病类型方面,牙髓炎50例(牙髓炎组)、根尖周炎50例(根尖周炎组)、牙髓坏死50例(牙髓坏死组)。对三组患者采用根管治疗术一次法治疗,观察术后7 d内疼痛的发生情况及术后6个月及1年内的整体临床治疗效果,并进行对比分析。结果:根尖周炎组患者治疗7 d后的疼痛发生率为20.0%,稍低于牙髓坏死组和牙髓炎组,但差异无统计学意义(P>0.05),同时牙髓坏死组、牙髓炎组患者疼痛发生率差异也无统计学意义(P>0.05);根尖周炎组患者在治疗后6个月及1年的成功率分别为92.0%和92.0%,稍高于牙髓炎组和牙髓坏死组,但差异无统计学意义(P>0.05),同时牙髓炎组和牙髓坏死组患者的成功率差异也无统计学意义(P>0.05)。结论:对于第一磨牙牙髓炎、牙髓坏死及根尖周炎疾病采用根管治疗术一次法治疗,临床效果良好,成功率高,能减少患者的就诊次数,缩短疗程,值得临床推广应用。
Objective： To explore clinical effect of root canal treatment on pulpitis, dental pulp necrosis and root tip periarthritis of mandibular first molar. Method： From November 2014 to March 2016 in the author＇s hospital 150 patients with periarthritis of root disease of mandibular first molar, a total of 150 teeth were selected, in terms of disease type, 50 cases of pulpitis（pulpitis group）, 50 cases of periarthritis of root（root periaithritis group）, 50 cases of dental pulp necrosis（pulp necrosis group）.Mter treatment of visit root canal therapy, occurrence of pain and the overall clinical therapeutic effect were analyzed.Result： Pain incidence of root periarthritis group after 7 days was 20.0%, slightly lower than the pulp necrosis group and pulpitis group, but there was no significant difference（P〉0.05）, at the same time, incidence of pain in patients of pulp necrosis group and pulpitis group dad no significant difference（P〉0.05）.The success rate of root periarthritis group after treatment of 6 months and 1 year were 92.0% and 92.0% respectively, slightly higher than that of pulpitisgroup and pulp necrosis group, but there was no statistically significant difference（P〉0.05）, success rate of pulpitis group and pulp necrosis group hadno statistical significance（P〉0.05）.Conclusion： Treatment of pulpitis, pulp necrosis and root tip periarthritis of the first molar pulpitis by using root canal treatment can short treatment period with good effect and high success rate, patients suffered the pain is small, so it is worth of wide application in clinic.
