摘要
目的:探讨二维两切面滑动扫查联合三维/四维超声在诊断胎儿手足畸形中的应用价值。方法:选取2010年1月-2016年12月在枣庄市妇幼保健院做产前超声筛查的中孕期胎儿。先由一位主治医师利用传统二维超声检查胎儿手足结构,记录检查结果,再由另一位主治医师利用二维两切面滑动扫查联合三维/四维超声检查胎儿手足结构并记录检查结果,以引产或出生后胎儿的外观手足畸形或影像学检查结果作为金标准,计算出两种检查方法对胎儿手足结构畸形的检出率。结果:共对31 177例胎儿进行了筛查,经引产或产后证实手足畸形70胎,畸形率为0.22%(70/31 177);畸形部位87处,传统二维超声检出手足畸形部位75处,检出率为86.21%,二维两切面滑动扫查联合三维/四维检出畸形部位85处,检出率为97.70%,两种方法检出率比较,差异有统计学意义(χ~2=8.037,P=0.005)。40胎手足异常胎儿接受了染色体检查,结果显示12胎染色体异常,其中18三体占75.00%(9/12),13三体占16.67%(2/12),21三体占8.33%(1/12),染色体异常胎儿中大部分为多处畸形,占91.67%(11/12)。结论:二维两切面滑动扫查联合三维/四维能提高对胎儿手足畸形的检出率。
Objective:To explore the application value of two-dimensional two plane with sliding scanning combined three-dimensional/four-dimensional ultrasound in the diagnosis of fetal limb abnormalities.Method:Fetal in middle pregnancy received prenatal ultrasound from January 2010 to December 2016 in Zaozhuang Maternity Child Health Care Hospital were selected.One visiting staff used traditional twodimensional ultrasonic check the fetal extremities structure and recorded the results,another visiting staff used two-dimensional two plane with sliding scanning combined three-dimensional/four-dimensional ultrasound checked the fetal extremities structure and recorded the results too,the appearance or imaging examination results of limb abnormalities after birth or induced labor act as the gold standard,calculated the detection rate of two inspection methods in checking of fetal limb abnormalities.Result:A total of 31 177 fetuses were studied,70 cases of limb abnormalities were confirmed by induced labor or postpartum,the deformity rate was 0.22%( 70/31 177);87 of deformed parts were confirmed by induced labor or postpartum births,traditional two-dimensional ultrasonic detect deformed parts 75,the detection rate 86.21%;two-dimensional two plane with sliding scanning combined threedimensional/four-dimensional detected deformed parts 85,the detection rate 97.70%,the detection rates of the two methods were compared,the difference was statistically significant(χ^2=8.037,P=0.005).40 fetuses with limb abnormalities accepted chromosome examination,the results for 40 chromosome analysis were as follows:12 cases were abnormal,75.00%(9/12) for trisomy 18,16.67%(2/12) for trisomy 13 and 8.33%(1/12) for trisomy 21,most cases of the chromosome abnormalities about 91.67%(11/12) were multiple malformations.Conclusion:Two-dimensional two plane with sliding scanning combined three-dimensional/four-dimensional can improve the detection rate of fetal limb abnormalities.
