摘要
目的分析快速康复理念在肩袖损伤关节镜下微创手术围术期护理及功能训练中的应用效果。方法将100例行关节镜下微创手术的肩袖损伤患者随机分为对照组(常规护理)和观察组(快速康复指导护理及功能训练)。以术前、术后5周、10周、15周、20周为观察时间点、比较两组Constant Murfey肩关节评估中各项指标(疼痛、肌力、关节活动度、日常生活)分值。比较两组焦虑量表(SAS)、抑郁量表(SDS)分值及护理总有效率。结果术前,两组患者疼痛、肌力、ROM(关节活动度)、日常生活分值组间比较均无明显差异;观察组术后10周起疼痛分值较对照组低、术后各观察时间点肌力、日常生活、ROM均高于对照组(P<0.05);干预后观察组SDS及SAS分值均低于对照组观察组护理总有效率高于对照组(P<0.05)。结论快速康复理念用于肩袖损伤围术期护理及功能椴炼中具有以下作用:①改善患者心理状态,使其积极配合治疗和康复;②缓解疼痛,利于功能训练顺利进行;③早期进行功能训练,从而提高肌力和关节活动度,改善日常生活,效果显著。
Objective To analyze the effect of rapid rehabilitation concept in perioperative nursing of arthroscopic mini- mally invasive surgery and functional training in rotator cuff injury. Methods A total of 100 patients undergoing arthroscopie minimally invasive operation were randomly divided into control group （with routine nursing） and observa tion group （with rapid rehabilitation guidance and functional training）. The scores of indicators including pain, muscle strength, joint activity, and daily life in Constant Murley shoulder joint assessment were compared between the two groups at the time of before operation,5 weeks, 10 weeks, 15 weeks and 20 weeks after operation. Anxiety scale （SAS）, depression scale（SDS） scores and nursing total effective rate between the two groups were compared. Results There were no significant differences in scores of pain, muscle strength, ROM（joint activity） and daily life between the two groups before operation,. Pain scores in the observation group were lower than those in the control group since 10 weeks after operation.Muscle strength, daily life and ROM were all higher than those in the control group at each observation time point（P〈0.05）. After treatment, the scores of SDS and SAS in the observation group were lower than those in the control group （P〈0.05）. The total effective rate was higher in the observation group than that in the control group （P〈0.05）. Conclusion The concept of rapid rehabilitation uesed in perioperative care and functional exercise Of the rotator cuff injury has the following effects：firstly,it can improve the patient＇s mental state to make the patient actively treated and rehabilitated;secondly,it can relieve pain, so as to facilitate the smooth operation of functional training;thirdly,early functional training can be taken, thereby improving muscle strength and joint mobility to improve daily life with a sig- nificant effact.
