摘要
目的:观察五虎汤合葶苈大枣泻肺汤加减治疗新生儿支原体肺炎痰热闭肺证的临床疗效。方法:93例新生儿支原体肺炎痰热闭肺证患儿随机分为观察组47例,对照组46例。对照组患儿给予常规治疗方案,观察组患儿在对照组治疗基础上给予五虎汤合葶苈大枣泻肺汤加减辨证治疗。比较两组患儿临床疗效、临床症状改善时间、血清β2微球蛋白(β2-microglobulin,β2-MG)以及外周血白细胞介素-10(interleukin-10,IL-10)、IL-17以及转化生长因子-β1(transforming growth factorβ1,TGF-β1)变化情况以及相关不良反应。结果:观察组患儿有效率97.87%,显著高于对照组82.61%,差异有统计学意义(P<0.05);观察组患儿退热时间、咳嗽明显减少时间以及啰音消失时间均显著短于对照组(P<0.05);治疗后两组患儿血清β2-MG,外周血IL-10、IL-17及TGF-β1均显著降低,且观察组患儿降低更显著,差异有统计学意义(P<0.05);两组患儿不良反应发生率比较,无显著差异(P>0.05)。结论:五虎汤合葶苈大枣泻肺汤加减辨证治疗新生儿支原体肺炎痰热闭肺证临床疗效显著,可改善患儿临床症状,降低患儿肾功能损伤,改善患儿机体炎症反应。
Objective ：To analysis the clinical efficacy of Wuhu Decoction combined with Tingli Dazao Xiefei Decoction in treatment of neonatal mycoplasma pneumonia with Phlegm-Heat Obstructing Lung Syndrome. Methods：Ninety-three cases of neonatal mycoplasma pneumonia with Phlegm-Heat Obstructing Lung Syndrome treated in our hospital from February 2014 to February 2016 were selected and were randomly divided to observation group （47 cases） and control group （46 cases）. The patients in the control group were given conventional treatment, and the patients in the observation group were treated with Wuhu Decoction combined with Tingli Dazao Xiefei Decoction treatment in addition to that of the control group. The levels of serum β2-mieroglobulin （ β2- MG）, interleukin-10 （IL-10） ,interleukin-10 （ IL-17 ） and transforming growth factor （IL-17） （TGF-131） and the related adverse reactions of the both groups were compared. Results：The effective rate of the observation group was 97.87 %, which was significantly higher than that （ 82.61% ） of the control group（ P 〈 0.05 ） ; The break down time of fever, the reducing time and the rales disappearing time of cough of the observation group were significantly shorter than that of the control group （ P 〈 0.05 ）. After treatment, the levels of serum β2-MG,IL-10, IL-17 and TGF-β1 in both groups significantly decreased, and the decrease of that of the observation group was more statistically significant （ P 〈 0.05 ）. There was no significant difference in the incidence of adverse reactions between the both groups （P 〉 0.05 ）. Conclusion ： Wuhu Decoction combined with Tingli Dazao Xiefei Decoction in treating for neonatal mycoplasma pneumonia has obvious clinical curative effect, which can improve clinical symptoms, reduce renal function damage and improve the inflammatory reaction in children.
