摘要
目的:探讨儿童肝功能指标中血清丙氨酸氨基转移酶(ALT)水平升高的病因及临床分析。方法:对1 627例ALT异常患儿的临床资料进行回顾性分析。结果:男性多于女性,其中<1岁849例,1~2岁组245例,3~5岁组229例,6~12岁组260例,13~16岁组44例。感染性疾病709例,其中病毒感染566例,以巨细胞病毒、EB病毒、呼吸道合胞病毒最常见;非感染性疾病471例,以结缔组织病、外伤、肝胆系统结构异常最常见;不明原因组447例。黄疸126例,肝大472例,脾大204例,淋巴结肿大180例,出血倾向51例。结论:婴儿是ALT异常主要人群。感染尤其病毒感染是ALT异常的主要原因。非感染性疾病中结缔组织病最常见。此外,外科疾病、药(毒)物、遗传代谢性疾病等均能致ALT异常。
Objective： To investigate etiology and clinical analysis in children with alanine aminotrasferase（ALT） rising. Methods：The data of hospitalized children with ALT abnormalities were statistical analyzed retrospectively. Results： The number of males was significantly more than females. In these children of ALT abnormalities,849 cases were infants under one year old,245 cases were between 1 year to 2 years old,229 cases were between 3 to 5 years old,260 cases were between 6 to 12 years old,44 cases were between 13 to 16 years old. A total of 709 cases were attributed to infection,which was the main cause of ALT abnormality. Five hundreds and sixty six cases were virus infected,Cytomegalovirus（CMV）,Epstein Barr virus（EBV） and Respiratory syncytial virus（RSV） infection were the most common etiology. Then,in non-infective diseases,ALT abnormality was more commonly resulted by connective tissue diseases,trauma,and liver or biliary duct structure abnormalities. However,the etiologies of 447 cases were still unknown. In this study,126 cases showed jaundice,472 cases showed hepatomegaly,204 cases showed splenomegaly,180 cases showed lymphadenovarix,and 51 cases showed bleeding tendency. Conclusion： Infants were the main groups of ALT abnormality.Infection,especially virus infection was the most common etiology. In non-infective diseases,connective tissue disease was more common. In addition,surgical disease,drug substance,hereditary metabolic disease,and so on,can lead to abnormal ALT.
