摘要
目的探讨棒球式缝合止血法在妊娠合并子宫肌瘤剔除术的临床应用,为临床手术方案的选择提供参考。方法选取2013年1月-2016年1月在东莞市第八人民医院接受妊娠合并子宫肌瘤剔除术的患者59例,随机分为观察组30例和对照组29例。对照组患者采用连续褥式缝合法缝合子宫创面,观察组患者采用棒球式缝合止血法缝合子宫创面,对比两组患者的手术时间、术中出血量、术后体温恢复正常时间、手术前后血红蛋白差值以及住院时间,同时对比两组患者术后并发症的发生情况。结果观察组患者手术时间(63.02±19.76)min、术中出血量(70.96±13.86)ml、术后体温恢复时间(1.63±0.96)d、手术前后血红蛋白差值(5.92±3.69)g/L、住院时间(6.02±1.97)d均显著少于对照组的(86.17±25.81)min、(118.39±30.52)ml、(3.88±1.42)d、(7.83±4.17)g/L、(8.15±5.57)d,两组比较差异均有统计学意义(P<0.05);两组患者术后并发症发生率,差异无统计学意义(P>0.05)。结论棒球式缝合止血法可减少妊娠合并子宫肌瘤剔除术的出血量,缩短患者术后的康复时间,且安全性高,具有良好的临床应用价值。
Objective To explore the clinical application of baseball suture hemostasis in myomectomy among pregnant women,provide reference for selection of clinical surgery programs.Methods Fifty-nine pregnant women underwent myomectomy in the hospital from January 2013 to January 2016,then they were randomly divided into observation group（30 cases） and control group（29 cases）.The patients in control group were treated by continuous mattress suture,while the patients in observation group were treated by baseball suture hemostasis.The operation time,the amount of blood loss during operation,postoperative body temperature recovery time,hemoglobin difference value between before and after surgery,hospitalization time,and the incidence rates of postoperative complications were compared between the two groups.Results In observation group,the operation time was（63.02 ± 19.76） minutes,the amount of blood loss during operation was（70.96± 13.86） ml,the postoperative body temperature recovery time was（1.63 ± 0.96） days,the hemoglobin difference value between before and after surgery was（5.92 ± 3.69） g/L,the hospitalization time was（6.02 ± 1.97） days,which were statistically significantly lower than those in control group [（86.17±25.81） minutes,（118.39±30.52） ml,（3.88±1.42） days,（7.83±4.17） g/L,（8.15±5.57） days]（P〈0.05）.There was no statistically significant difference in the incidence rates of postoperative complications between the two groups（P〉0.05）.Conclusion Baseball suture hemostasis can reduce the amount of bleeding,shorten the recovery time of patients after operation with high safety,which has high clinical application value.
