呼吸训练对神经内镜下切除垂体瘤患者内分泌激素、肺功能与生存质量的影响 被引量:7

摘要 目的探讨呼吸训练对神经内镜下切除垂体瘤患者内分泌激素、肺功能与生存质量的影响。方法选取我院神经内科2013年1月至2014年1月收治的垂体瘤患者60例,按数字表随机法分为观察组和对照组,每组30例,在神经内镜下切除垂体瘤术后,对观察组患者进行1个星期的呼吸训练,对照组患者则不进行呼吸训练,比较两组患者呼吸训练前后内分泌激素、肺功能及患者生存质量的情况。结果手术切除垂体瘤后,两组患者的促甲状腺激素、血清游离三碘甲腺原氨酸、血清游离甲状腺素、促肾上腺皮质激素、血浆总皮质醇等水平均较训练前有所升高,差异均有统计学意义(P<0.05),且观察组的上述激素水平升高幅度大于对照组,但两组各激素水平比较差异均无统计学意义(P>0.05);术后呼吸训练1个星期后,观察组患者在肺活量(VC)、第一秒用力呼气量(FEV1)、用力肺活量(FVC)、第一秒用力呼气量占用力肺活量的百分率(FEV1/FVC)及每分钟最大通气量(MVV)等肺功能指标明显优于对照组,差异均有统计学意义(P<0.05);术后呼吸训练1个星期后,观察组患者的SF-36生存量表平均得分明显高于对照组,差异有统计学意义(P<0.05)。结论呼吸训练对神经内镜下切除垂体瘤患者的内分泌激素水平没有影响,但对患者肺功能的恢复有重要作用,同时也可提高患者的生存质量,值得临床推广应用。 Objective To evaluate the impact of respiratory training on patients' endocrine hormone, lung function and life quality after endoscopic pituitary adenoma resection. Methods Sixty patients with pituitary adenoma were selected in our hospital from January 2013 to January 2014 and randomly divided into two groups. After endoscopic resection of pituitary adenoma, the experimental group(n=30) received respiratory training for one week, and the control group(n=30) did not receive respiratory training. The patients' endocrine hormone, lung function, and life quality were compared between the two groups. Results After endoscopic resection of pituitary adenoma, the levels of serum thyroid-stimulating hormone(TSH), free three iodine thyroid original glycine(FT3), free thyroxine, adrenocorticotropic hormone(ACTH), total plasma cortisol after training in the two groups were significantly higher than those before the training(P〈0.05). And the increase of the hormone levels in observation group were greater than that in control group, but the differences were not statistically significant(P〉0.05). After one week of respiratory training, the vital capacity(VC),forced expiratory volume in one second(FEV1), forced vital capacity(FVC), FEV1/FVC, and maximum ventilatory volume/minute(MVV) in the observation group were significantly better than those in the control group(P〈0.05). After one week of respiratory training, the average score of SF-36 in the observation group was significantly higher than that in the control group(P〈0.05). Conclusion Respiratory training plays an important role in the recovery of pulmonary function and can improve the quality of life of patients, without affecting patients' endocrine hormone levels, which is worthy of clinical promotion.
