摘要
目的探讨Wnt通路相关蛋白在胃癌前病变发生、发展中的作用以及在胃癌早期诊断中的意义。方法选取2012年6月—2015年6月胃癌前病变胃镜活检标本256例(20例轻度异型增生、44例中度异型增生、52例重度异型增生、140例肠上皮化生),同时选取45例正常胃黏膜组织标本作为对照组。采用免疫组织化学方法对各组组织标本中Wnt通路相关蛋白Wnt1、β-连环蛋白(β-catenin)、结肠腺瘤性息肉病蛋白(APC)和细胞周期蛋白D1(cyclin D1)蛋白表达水平进行检测。分析各组Wnt通路相关蛋白的表达水平及阳性表达率。结果各观察组Wnt1、β-catenin和cyclin D1蛋白表达水平高于对照组,且APC蛋白水平显著低于对照组(P<0.05)。中度和重度异型增生组Wnt1、β-catenin和cyclin D1蛋白表达水平高于轻度异型增生组和肠上皮化生组,而APC蛋白水平低于轻度异型增生组和肠上皮化生组(P<0.05)。重度异型增生组Wnt1、β-catenin和cyclin D1蛋白表达水平高于中度异型增生组,而APC蛋白水平低于中度异型增生(P<0.05)。与对照组比较,Wnt1、β-catenin和cyclin D1在各观察组中的阳性表达率增加(P<0.05),APC阳性表达率下降(P<0.05);中度和重度异型增生的各蛋白阳性表达率与轻度异型增生组差异比较具有统计学意义(P<0.05)。结论 Wnt通路相关蛋白在胃癌前病变组织中呈异常表达,从而激活了Wnt通路,参与了胃癌的发生和发展,对其进行联合检测有助于胃癌的早期诊断。
Objective To investigate effect of Wnt pathway related proteins in pathogenesy and development of precancerous lesions of gastric cancer and its significance in early diagnosis. Methods A total of 256 endoscopic biopsy samples of precancerous lesion of gastric cancer (20 cases of mild dysplasia, 44 cases of moderate dysplasia, 52 cases of severe dysplasia and 140 cases of intestinal metaplasia) during June 2012 and June 2015 were chosen as the observation group, and 45 cases of normal gastric mucosa tissues were selected as control group simultaneously. Protein levels of Wnt pathway related proteins Wntl, β-catenin, activated protein C (APC) and cyelin D1 expressions in the two groups were detected by immunohistochemistry method. The expression levels of Wnt pathway related proteins and positive expression rates were analyzed. Results Expression levels of Wntl, β-catenin and cyclin D1 proteins were significantly higher, and the levels of APC protein in observation groups were significantly lower than those in control group (P 〈 0.05). Ex- pression levels of Wntl, β-catenin and cycfin D1 proteins were significantly higher, while APC protein levels were significantly lower in the moderate and severe dysplasia groups than those in mild dyspiasia and intestinal metaplasia groups (P 〈0.05). Expression levels of Wntl, β-eatenin and cyclin D1 proteins were significantly higher, while APC protein level was significantly lower in severe hyperplasia group than those in moderate dysplasia group ( P 〈 0.05 ). Compared with those in control group, the positive expression rates of Wntl, β-catenin and cyelin D1 proteins were significantly in- creased ( P 〈 0.05 ) , while APC positive expression rates were significantly decreased in observation groups ( P 〈 0. 05 ). There were statistically significant differences in positive expression rate of proteins between moderate and severe dysplasia groups and mild dysplasia group (P 〈 0. 05 ).
