摘要
目的:本研究拟通过改良的质量调整生命年来评估外科择期手术患者实施贮存式自身输血(PABD)的临床疗效和实用性,研究制定科学合理的PABD实施方案,有效节约血液资源,为临床贮存式自身输血治疗提供科学指导。方法:选取符合纳入/排除标准并自愿进行PABD的外科择期手术患者200例为研究对象,经过PABD适合度的评估,以评估级别,采用随机法,随机分配成试验组和对照组,每组100例,试验组为PABD组,对照组为异体输血组。用改良的质量调整生命年(IQALY)来分析2组住院时间、不良反应、术后感染及预后情况。评估PABD的临床疗效和实用性,为临床开展贮存式自身输血治疗提供科学可靠有效的依据。结果:在IQALY的分析应用中,我们将较小的值定义为较为优良的生活质量;用IQALY计算得出值:IQALY试验组=16.67;IQALY对照组=34.00。结论:进行PABD的患者,预后情况较好、平均住院天数更少,在手术后输血不良反应和感染少,临床医生在对择期手术患者进行输血治疗时,建议考虑这些后续的效应和作用。
Objective：To assess the clinical efficacy and practicality of preoperative autologous blood donation （PABD） in the surgical patients undergoing elective surgery by improved quality-adjusted life year （IQALY）, pro- vide scientific, reliable and effective basis for clinical transfusion therapy, and save blood resources effectively. Method： We selected 200 elective surgical patients who met the inclusion/exclusion criteria and voluntarily signed informed consent PABD for the study, then used a randomized control method. After PABD fitness assessments, based on the assessment level, these patients were randomly assigned to the experimental group and a control group by stratified random allocation method, which group included 100 patients. The patients underwent PABD in experimental group, and the others transfused allogeneic blood. IQALY was used to analysis data. Results：We defined the less number was the better condition in IQALY. The value showed： IQALYexperimental = 16.67; IQALYcontrol = 34.00. Conclusion： Patients undergoing PABD of the experimental group have less average days of hospitalization, fewer adverse reactions, less post-operative infections and better prognosis. The total saved blood volume is huge, and the saved cost of blood transfusion is large. Clinicians should take into account these effects when patients undergoing elective surgery need blood transfusion therapy.
