摘要
目的分析经胸超声心动图（TTE）在妊娠合并感染性心内膜炎（IE）临床诊断中的价值。方法回顾性分析2009年12月至2015年12月首都医科大学附属北京安贞医院收治的5例妊娠合并IE患者临床资料，分析5例患者的临床特征、心脏基础疾病、临床治疗结果、TTE特征。结果5例妊娠合并IE患者中患先天性心脏病3例，瓣膜疾病2例；TTE检出赘生物5例，其中误诊1例。检出赘生物包括主动脉瓣赘生物、肺动脉瓣赘生物、三尖瓣赘生物、人工机械瓣赘生物。5例患者中4例行剖宫产术终止妊娠，4例新生儿均存活，1例行剖宫取胎术终止妊娠；5例患者均行心脏手术治疗，均痊愈出院。结论TTE对妊娠合并IE赘生物形成有较高的检出率，对IE的临床诊断及指导治疗均有重要意义。
ObjectiveTo analyze the value of transthoracic echocardiography（TTE） in diagnosis of pregnancy complicated with infective endocarditis（IE）. MethodsFive pregnant women with IE from December 2009 to December 2015 in Beijing Anzhen Hospital, Capital Medical University were retrospectively enrolled. Clinical characteristics, heart disease, therapeutic effect and TTE features were analyzed. ResultsThree patients had congenital heart disease and 2 patients had valvular heart disease. TTE showed vegetation in all 5 patients, but 1 case was misdiagnosed; vegetations distributed on aortic valves, pulmonary valves, tricuspid valves and artificial mechanical valves. Four patients had cesarean section, operations were successful. One patient had therapeutic abortion. Five patients had cardiac surgery and recovered well. ConclusionsTTE has good significance in diagnosis and treatment of IE.
