盐酸右美托咪定对甲状腺手术患者围术期血糖、IL-6和TNF-α的影响分析

摘要 目的:探讨盐酸右美托咪定对甲状腺手术患者围术期血糖、IL-6和TNF-α的影响,为临床应用提供参考。方法:选取我院2014年1月~2016年收治甲状腺手术患者68例,随机分为研究组和对照组。研究组34例,前10min,给予盐酸右美托咪定0.8μg/kg负荷剂量静脉输注,后以0.3~0.6μg/(kg.h)维持静脉输注;对照组34例,给予等量生理盐水维持静脉输注。两组均于麻醉诱导前10min(T0)、术毕即刻(T1)、术后6h(T2)及术后24h(T3)四个时间点抽取外周静脉血,测定血糖、血清IL-6和TNF-α,比较组间差异。结果:T0时,两组各项指标无显著差异,P>0.05;对照组中,T1、T2、T3时,血糖水平、血清IL-6和TNF-α均显著高于T0时,差异具有统计学意义(P<0.05),且均大于研究组对应时刻(P<0.05);研究组中,T1、T2、T3时各项指标无明显变化(P>0.05)。结论:右美托咪定应用于甲状腺手术患者围术期,可有效缓解血糖升高,抑制机体应激反应,血流动力学更加平稳,值得临床推广。
