地佐辛复合罗哌卡因对胃癌患者术后镇痛及机体应激反应免疫功能的影响 被引量:10

Effect of Dezocine Combined with Ropivacaine on Postoperative Analgesia,Stress Response and the Immune Function of Patients with Gastric Cancer

摘要 目的:探索地佐辛复合罗哌卡因对胃癌患者术后镇痛及机体应激反应、免疫功能的影响。方法:研究在我院行胃癌手术的患者74例,根据患者使用药物不同分为地佐辛复合罗哌卡组(研究组)和芬太尼复合罗哌卡因组(对照组),分析比较两组患者术后镇痛及机体应激反应、免疫功能的差异等。结果:两组患者术后4h、术后8h、术后24h和术后48h的视觉模糊评分(VAS)量表得分研究组低于对照组(P<0.05)。手术后4h、12h和24h研究组的血浆内皮素(ET)和血管紧张素Ⅱ(AngⅡ)水平均低于对照组(P<0.05)。手术后24h,研究组的CD3、CD4和NK细胞水平高于对照组(P<0.05)。术后研究组的总不良反应低于对照组(P<0.05)。结论:地佐辛复合罗哌卡因对胃癌患者术后的镇痛效果较好,能够降低机体应激反应水平,增强患者的免疫功能,临床应用效果更优。 Objective: To explore the effects of Dezocine combined with Rropivacaine on postoperative analgesia,stress response and the immune function of patients with gastric cancer. Methods: 74 cases of gastric cancer patients in our hospital undergone surgery were divided into dezocine combined with ropivacaine group( study group) and fentanyl ropivacaine group( control group) according to different drugs used by the patients. Postoperative analgesia,stress reaction and immune function of the two groups were compared. Results: 4h,8h,24 h and 48 h after the operation,the scale scores of the patients from visual fuzzy scale( VAS)in the study group were significantly lower than those in the control group( P〈0.05). 4h,8h and 24 h after the operation,the levels of ET and Ang Ⅱ in the study group were significantly lower than those in the control group( P〈0.05). 24 h after the operation,the levels of CD3,CD4 and NK cells in the study group were significantly higher than those in the control group( P〈0.05). The total adverse reactions after the operation in the study group were significantly lower than those in the control group( P〈0. 05). Conclusion: Dezocine combined with Ropivacaine has better analgesia effect in postoperative patients with gastric cancer,can reduce patients' stress response level and enhance patients' immune function,whose clinical effect is better.
