摘要 目的探讨肺表面活性物质(PS)不同给药方法治疗新生儿肺透明膜病(HMD)的临床效果及对血气指标的影响。方法选取2014年9月~2016年3月中国石油天然气集团公司中心医院儿科收治的120例新生儿HMD患儿作为研究对象,采用随机数字表法分为对照组(n=60)和观察组(n=60)。两组均在常规治疗的基础上给予PS治疗,对照组采用传统气管导管给药,观察组采用改良给药方法,比较两组的临床疗效,治疗前后血氧分压-肺泡氧分压(a/APO_2)、氧分压(PaO_2)、二氧化碳分压(PaCO_2)、pH,并发症,给药时间和住院时间。结果观察组总有效率明显高于对照组,差异有统计学意义(P<0.05)。观察组治疗后a/APO_2、PaO_2明显高于对照组,PaCO_2明显低于对照组,差异有统计学意义(P<0.05);两组治疗后p H比较,差异无统计学意义(P>0.05)。观察组并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。观察组给药时间和住院时间均明显短于对照组,差异有统计学意义(P<0.05)。结论改良给药方法治疗新生儿HMD的临床效果突出,显著改善患者的血气指标,值得临床推广应用。 Objective To evaluate the effect of different administration methods of pulmonary surfactant (PS) in the treatment of neonatal pulmonary hyaline membrane disease (HMD) and the influence on blood gas index. Methods 120 children with neonatal pulmonary HMD from September 2014 to March 2016 in Department of Pediatrics, Central Hospital of China National Petroleum Corporation were selected as study objects. All patients were divided into control group (n=60) and observation group (n=60) by random number table method. PS treatment on the basis of conventional therapy in two groups. Control group was treated with traditional endotracheal tube, observation group was treated with modified administration method. Clinical effect, a/APO2, PaO2, PaCO2, pH before and after treatment, complication, ad- ministration time and length of hospital stay between two groups were compared. Results The effective rate of observa- tion group was significantly higher than that of control group, with statistical difference (P 〈 0.05). After treatment, a/ APO2 and PaO2 of observation group were higher than those of control group, PaCO2 of observation group was lower than that of control group, with statistical differences (P 〈 0.05). After treatment, pH in two groups was compared, with no statistical difference (P 〉 0.05). Incidence rate of complication in observation group was significantly lower that in control group, with statistical difference (P 〈 0.05). Administration time and length of hospital stay in observation group were shorter than those in control group, with statistical difference (P 〈 0.05). Conclusion Clinical effect of modified administration method in the treatment of neonatal pulmonary HMD is distinct, significantly improve blood gas index of patient, it is worthy of clinical promotion and application.
