经食管三维超声在房间隔缺损封堵术术前评估中的应用 被引量:9

摘要 目的探讨经食管实时三维超声心动图(RT3D-TEE)在房间隔缺损(ASD)封堵术前评估中的应用价值。方法38例经胸二维超声初诊为继发孔型ASD的患者行RT3D-TEE检查,观察缺损的形态测量最大径、面积及残缘,与经胸二维法比较。筛选可封堵的病例,将缺损最大径与封堵器型号作相关性分析。结果RT3D-TEE显示35例ASD为继发孔型,其中20例为类椭圆形,13例伴软缘,16例前缘缺乏;另外3例为非继发孔型不可封堵。二、三维法测量的缺损最大径、上下残缘为差异有统计学意义(P<0.05),其余残缘差异无统计学意义(P>0.05),缺损最大径与封堵器型号均显著相关(r=0.906、0.976,P<0.05),但三维法与之相关性更高。结论RT3D-TEE能清晰显示ASD的三维解剖精准测量缺损口及残缘是封堵术前评估的重要方法。 Objective To explore the value of real-time three-dimensional transesophageal echocardiography (RT3D-TEE) in evaluating atrial septal defect (ASD) before closure. Methods A total of 38 patients, who had been diagnosed with secundum ASD by two-demensional transthoracic echocardiography (2D-TTE) previously, were given RT3D-TEE examination. The morphologic features of ASD including maximal diameter, area and the length of surrounding rims were measured by RT3D-TEE, and the results were compared with those obtained by 2D-TTE. Appropriate candidates were recognized for closure, and the correlation between maximum diameter of ASD and size of closure device were analyzed. Results RT3D-TEE showed that 35 cases dignosed as secundum ASD, 20 cases as oval ASDs, and 13 cases had soft rims, 16 cases lacked of front rim. Another 3 patients were not suitable for closure, who were diagnosed with non-secundum ASD. There were significant diffrences of the maximal diameter of ASD and the length of superior or inferior rims measured between the two methods ( P〈0.05), and there was no significant diffrence in the length of other rims ( P〉0.05). The correlation between the maximum diameter of ASD and the size of closure device were intimate significantly ( r= 0.906 and 0.976, P〈 0.05), but RT3D-TEE was higher. Conclusions RT3D-TEE can clearly display three-dimensional anatomic features of ASD, accurately measure the sizes of ASD and surrounding rims. Therefore, RT3D-TEE play an important role in evaluating ASD before closure.
