摘要
目的探讨血清同型半胱氨酸浓度在与各期慢性肾脏病的表达及对分期的敏感性。方法实验组,选取诊断为慢性肾脏病(CKD)患者100例,根据肾小球滤过率(e GFR),分别选取CKD 1期、CKD 2期、CKD 3期、CKD 4期、CKD 5期非透析患者各20例;对照组,选取某医院体检中心健康体检者20例。分别测定血清尿素氮、肌酐以及同型半胱氨酸的浓度,采用SPSS15.0软件进行统计学分析。结果 CKD 1期、CKD 2期、CKD 3期患者与对照组血清同型半胱氨酸水平差异无统计学意义(P>0.05),CKD 4期与CKD 5期患者血清中同型半胱氨酸的水平高于对照组,差异有统计学意义(P<0.05);各期患者血清同型半胱氨酸之间相比较,CKD 1期低于CKD 3期、CKD4期、CKD 5期的水平,差异有统计学意义(P<0.05);CKD 2期和CKD 3期均低于CKD 5期的水平,差异有统计学意义(P<0.05);血肌酐及尿素氮在CKD 1期、CKD 2期中与对照组比较差异无统计学意义(P>0.05),在CKD 3期、CKD 4期、CKD 5期各组间比较差异均有统计学意义(P<0.05)。结论慢性肾脏病患者中血清同型半胱氨酸的浓度只有在CKD 4期和CKD 5期较健康体检者明显升高,不能作为早期肾损害的指标,在判断肾功能方面并不优于传统的血肌酐及尿素氮。
Objective To discuss the expression and diagnosis sensitivity of serum homocysteine in different stages of chronic kidney disease （CKD）. Methods Experimental groups： 100 patients who were diagnosed as CKD and dividedgrouped them into five groups according to different lomerular filtration rate （eGFR）, CKD1 （n = 20）, CKD2 （n = 20 ）, CKD3 （n = 20）, CKD4 （n = 20） and CKD5 （n = 20）, respectively. Control group：sing 20 cases of health people for physical examination were selected as control group. Collected the clinical data and measured such as the serum urea nitrogen （BUN） , creatinine （Scr）, and homocysteine （HHcy）） were analyzed. Results There were no significant differences in the level of HHcy between CKD1, CKD2, CKD3 and control groups （P ~ 0.05）, and in CKD4 and CKD5 groups, the HHcy was significantly higher than that in control group （ P 〈 0.05 ） ; Compared with stage CKD3, CKD4 and CKDS, the HHcy was lower in stage CKD1 （P 〈 0.05 ）, and more higher in stage CKD5 than that in stage CKD2 and CKD3 （ P 〈 0.05 ） ; There were no statistically difference in the level of BUN and Scr between stage CKD1, CKD2 and control groups （ P 〉 0.05 ）, but in stage CKD3, CKI）4 and CKDS, the concentration of BUN and Scr showed significant difference （P 〈 0.05）. Conclusion Comparing with the traditional tests such as Scr and BUN, the HHcy doesn＇t have any advantage in found of early kidney injury,which is only ele- vatinged in stage CKD4 and CKD5.
