经兔肝动脉灌注^131I标记CD147单克隆抗体治疗肝癌的实验研究 被引量:1

Transcatheter hepatic arterial infusion with ^131I-anti-CD147-monoclonal-antibody on treating liver cancer in a rabbit model

摘要 目的 评价经兔肝动脉灌注^131I标记CD147单克隆抗体(^131I-anti-CD147-McAb)治疗肝癌的有效性及安全性.方法 选取荷VX2肝癌的实验兔45只,随机数字表法均分为3组,均在全身麻醉下行经肝动脉灌注术.A组:对照组,灌注生理盐水.B组:单纯^131I组,灌注Na^131I溶液.C组:^131I-anti-CD147-McAb组,灌注^131I-anti-CD147-McAb溶液.分别于术前及术后1、3、7、14、21 d抽取3组实验兔静脉血约2 ml检测肝、肾、甲状腺功能及血常规.分别于术后1、7、14、21 d,对B、C组实验兔行单光子发射计算机体层成像(SPECT)-CT扫描,监测其体内核素分布情况及肿瘤大小.于术后14 d,每组随机数字表法抽取5只兔,处死行病理及免疫组织化学检查.各组剩余兔继续饲养,用于生存期分析.组间、组内计量资料比较采用重复测量方差分析,组间、组内两两比较采用LSD法,率的比较采用Mann-Whitney U检验,生存期以Kaplan-Meier生存分析法进行评估.结果 B、C组兔:术后1 d,丙氨酸转氨酶、天冬氨酸转氨酶升高,与术前相比差异有统计学意义(P〈0.05);术后7 d恢复至术前水平;其他肝功能指标无明显变化.术后1周,游离三碘甲状腺原氨酸、游离甲状腺素持续下降,同时血清促甲状腺素升高,与术前相比差异有统计学意义(P均〈0.05).术后白细胞计数略有下降,但与术前相比差异无统计学意义(P〉0.05).SPECT-CT示:术后1 d,B组兔核素显像大部分聚集于甲状腺及胃肠道,肝肿瘤内较少;C组兔核素显像主要聚集于VX2肿瘤内;14 d后,B、C两组实验兔体内各脏器均无核素显像.在治疗后,A、B组兔VX2肝肿瘤逐渐增大,C组肿瘤则逐渐缩小,3组肿瘤大小差异有统计学意义(P〈0.05);C组残瘤内微血管密度、基质金属蛋白酶2和血管内皮生长因子表达明显较A、B两组少(P均〈0.05),细胞凋亡明显较A、B组多,A、B、C组实验兔的中位生存时间分别为22、26和54 d, Objective To evaluate the efficacy and safety of treating liver cancer using ^131I-anti-CD147-monoclonal-antibody (^131I-anti- CD147-McAb) by transcatheter hepatic arterial infusion (TAI) in a rabbit liver cancer model. Methods Forty-five rabbit models were randomly divided into three groups evenly. Transcatheter hepatic artery infusion under general anesthesia were performed in all three groups. Group A:control group, saline. Group B:pure ^131I solution. Group C: ^131I-anti-CD147-McAb solution. About 2 ml blood sample was obtained from all rabbits for liver, kidney,and thyroid function at pre-TAI and post-TAI 1 day, 3 days ,7 days, 14 days, 21 days. The rabbits were scanned by single photon emission computed tomographic (SPECT) to monitor radionuclide bio-distribution and tumor size on 1 day, 7 days, 14 days, 21days after procedures in group B and C. On 14 days after procedure, five rabbits were randomly selected to be sacrificed in each group for pathological and immunological investigations. The remaining rabbits continued to be fed, and survival rates were measured. Results The TAI and SPECT-CT/CT procedures were successfully performed in all rabbits. Test results showed that AST and ALT levels tended to increase transiently 1 day after TAI (P〈0.05). The liver function returned to pre-operative levels (P〉 0.05) 7 days after TAI. FT3 and FT4 mean values of rabbits in group B and C continued to decline 7 days after TAI, while thyroid stimulating hormone (TSH) showed corresponding increase (P〈0.05). WBC mean value of rabbits in group B and C declined slightly after procedures (P〉0.05). SPEC-CT imaging of rabbits shows that most of the radionuclide was gathered in the gastrointestinal tract and thyroid in Group B, however, radionuclide was mainly concentrated in the tumor lesions in Group C.
