声触诊组织成像量化技术在诊断急性跟腱炎中的应用价值 被引量:6

摘要 目的探讨声触诊组织成像量化(VTIQ)技术在急性跟腱炎诊断中的应用价值。方法对8例急性跟腱炎患者的11条病变跟腱(急性跟腱炎组)和10例健康志愿者的20条跟腱(健康对照组)均行常规超声检查,测量其跟腱厚度;应用VTIQ技术测量跟腱的剪切波速度值。绘制受试者工作特征(ROC)曲线,确定VTIQ技术诊断急性跟腱炎的诊断界值。结果急性跟腱炎组跟腱厚度为(6.85±1.56)mm,较健康对照组(4.96±0.71)mm明显增加;剪切波速度值为(4.66±0.93)m/s,较健康对照组(9.04±0.70)m/s明显减低,两组比较差异均有统计学意义(P=0.002、0.000)。VTIQ技术诊断急性跟腱炎的最佳诊断界值为7.0 m/s,其曲线下面积为0.95,敏感性和特异性分别为90.0%和90.9%。结论 VTIQ技术能定性、定量评估跟腱的硬度,为临床诊断急性跟腱炎提供了一种简单、有效的方法。 Objective To explore the diagnostic value of virtual touch tissue imaging quantification(VTIQ) in the diagnosis of the acute achilles tendonitis.Methods Eight patients with acute achilles tendinitis(11 lesions achilles tendon,acute achilles tendinitis group) and 10 cases of healthy volunteers(20 achilles tendon,healthy control group)were underment routine ultrasound examination,measurement thickness of the achilles tendon flat to the medial malleolus place,the shear wave velocity value was measured by VTIQ technique,the receiver-operating characteristic(ROC)curve was drew,and confirmed the boundary values of VTIQ in the diagnosis of acute achilles tendinitis.Results The thickness of the acute achilles tendinitis group was higher than that of the healthy control group[(6.85±1.56)mm vs.(4.96±0.71)mm],shear wave velocity value was reduced[(4.66±0.93)m/s vs.(9.04±0.70)m/s],the difference had statistical significance(P=0.002,0.000).The best cut value of VTIQ technique in the diagnosis of acute achilles tendinitis was 7.0 m/s,the sensitivity and specificity were 90.0% and 90.9%.Conclusion VTIQ can evaluate the hardness of the acute achilles tendon qualitatively and quantitatively,it provides a simple and effective method for clinical diagnosis.
