摘要
目的探讨第二产程时间长短对顺产妇产后盆底肌力的影响。方法将2014年3月~2016年5月凌云县人民医院收治的200例产妇分为对照组(第二产程时间≤2 h,99例)和观察组(2 h<第二产程时间<3 h,101例)。产后6~8周采用法国PHENIX USB 4盆底肌肉刺激治疗仪检测两组产妇的盆底肌力。结果分娩后,观察组Ⅰ类肌纤维肌力损伤发生率为77.22%,高于对照组的57.57%,差异有统计学意义(P<0.01);观察组Ⅱ类肌纤维肌力损伤发生率为42.57%,稍高于对照组(41.41%),但差异无统计学意义(P>0.05)。观察组的Ⅰ、Ⅱ类肌纤维疲劳度分别为(9.3±0.8)%、(3.2±0.6)%,均高于对照组的(4.3±0.5)%、(2.6±0.4)%,差异有统计学意义(P<0.01)。观察组的Ⅰ、Ⅱ类肌纤维阴道压力分别为(62.4±7.4)、(79.3±2.5)cm H2O,均低于对照组的(81.5±8.6)、(92.3±1.2)cm H2O,差异有统计学意义(P<0.01)。结论第二产程的时间延长对盆底肌力的损伤更大,应对产妇进行及时的检查、预防。
Objective To explore the impact of the second birth course on pelvic floor muscle strength of puerpera after natural labor.Methods 200 puerperas from March 2014 to May 2016 in People′s Hospital of Lingyun County were divided into control group（the second birth course≤2 h,99 cases） and observation group（the second stage of labor more than 2 h and less than 3 h,101 cases）.French PHENIX USB 4 pelvic floor muscle stimulator was used to detected pelvic muscle strength of puerperal in two groups 6-8 weeks after delivery.Results After delivery,the incidence of type Ⅰmuscle fiber injury of observation group was 77.22%,was higher than that of the control group（57.57%）,the difference was statistically significant（P〈0.01）;the incidence of type Ⅱ muscle fiber injury of the observation group was 42.57%,was slightly higher than that of the control group（41.41%）,but the difference was not statistically significant（P〈0.05）.The fatigue of type Ⅰ and Ⅱ muscle fiber in the observation group was（9.3±0.8）% and（3.2±0.6）% respectively,was higher than that in the control group [（4.3±0.5）%,（2.6±0.4）%],the difference was statistically significant（P〈0.01）.The vaginal pressure of type Ⅰ and Ⅱ muscle fiber in the observation group was（62.4±7.4）,（79.3±2.5） cm H2 O respectively,was lower than that in the control group [（81.5±8.6）,（92.3±1.2） cm H2O],the difference was statistically significant（P〈0.01）.Conclusion The extension of the second stage of labor has more damage on pelvic muscle strength,and timely inspection and prevention should be done for puerperal.
