摘要
目的:分析聚合酶链式反应(PCR)与逆转录PCR(RT-PCR)检测咽拭子标本中肺炎支原体对比情况,为临床应用提供依据。方法:选取河北省中医院已采集咽拭子标本需检测肺炎支原体的患者328例的临床资料,临床确诊感染肺炎支原体患儿69例,疑似感染患儿259例,比较PCR与Rt-PCR检测结果阳性率、灵敏度、特异度。结果:328例患者的咽拭子标本中,PCR共检出肺炎支原体阳性标本33例,阳性率为10.06%。Rt-PCR共检出肺炎支原体阳性标本65例,阳性率为19.82%。Rt-PCR检测阳性率高于对照组,差异有统计学意义(P<0.05)。PCR的灵敏度(18.84%)比Rt-PCR的灵敏度(88.41%)低,且2者差异有统计学意义(P<0.05)。PCR的特异度(87.64%)比Rt-PCR的特异度(94.59%)低,且2者差异有统计学意义(P<0.05)。结论:在检测咽拭子中肺炎支原体时,Rt-PCR检测的阳性率、灵敏度以及特异度均优于PCR,值得在临床上推广。
Objective： To analyze the detection of mycoplasma pneumoniae in throat swab samples by polymerase chain reaction（ PCR） and reverse transcription PCR（ RT-PCR） and compare the results,so as to provide the basis for clinical application. Methods： Clinical data of 328 cases of patients in the TCM Hospital of Hebei Province from May 2011 to May 2016 were collected,of which69 cases were clinically diagnosed with mycoplasma pneumoniae infection and 259 cases were suspected to be infected. PCR and RtPCR test results were compared in terms of positive rate,sensitivity,and specificity. Results： In the 328 cases of throat swab specimens,33 cases of mycoplasma positive samples were detected by PCR,the positive rate being 10. 06%; 65 cases of positive samples were detected by Rt-PCR,with the positive rate of 19. 82%. The positive rate of Rt-PCR test was higher than that of the control group,and the difference was statistically significant（ P〈0. 05）. The sensitivity of PCR（ 18. 84%） was lower than that of Rt-PCR（ 88. 41%）,and the difference was statistically significant（ P〈0. 05）. The specificity of PCR（ 87. 64%） was lower than that of RtPCR（ 94. 59%）,and the difference was statistically significant（ P〈0. 05）. Conclusion： The positive rate,sensitivity and specificity of Rt-PCR in the detection of mycoplasma pneumoniae in throat swab are better than those of PCR. It is worth popularizing in clinical practice.
