摘要
[目的]总结导师范永升教授在临床上应用一贯煎治疗干燥综合征的经验,从而更好地指导临床实践。[方法]通过跟师临证和收集整理范老师应用一贯煎治疗干燥综合征的医案,学习范老师对干燥综合征病因病机的认识以及对一贯煎的方药分析,并列举范老师应用一贯煎治疗干燥综合征的两例经典病案。[结果]范老师认为干燥综合征的病机为阴虚为本,燥邪为标,不离肝郁,治疗上当以滋养肝肾之阴,兼以疏肝解郁为根本大法。范老师在干燥综合征的治疗过程当中以辨证论治为基础,根据实际临床症状、病因、病机的变化灵活应用一贯煎加减,或加用清热解毒、或活血化瘀、或健脾化湿、或益气养阴等方法,临床疗效满意。所举案例,取得较好效果。[结论]范永升教授一贯煎治疗干燥综合征的经验,值得学习并且在临床上推广应用。
[Objective] To summarize the experience of professor Fan Yongsheng who uses the herbal prescription“ Yiguanjian” to treat the sjojren syndrome,so as to direct our clincal practice better .[Method]By learning from professor Fan as well as collating his medical cases that applicating“ Yiguanjian”to the treatment of the sjojren syndrome, clearing and definiting how professor Fan thinks etiologies and pathogenesis of the sjojren syndrome and analyzing the prescription of“ Yiguanjian”and giving two typical clinical cases. [Result]Professor Fan thinks the pathogenesis of the sjojren syndrome is that yin deficiency and dryness pathogen as well as liver depression, while yin deficiency is the essential and dryness pathogen is the surface.During the treatment of the sjojren syndrome,the most important thing is nourishing the yin of liver and kidney as well as resolving liver depression. On the bases of syndrome differentiation and treatment,professor Fan uses the“ Yiguanjian”flexibly according to the different clinical syndromes, etiologies and pathogenesis of the sjojren syndrome, or combining other methods such as heat-clearing and detoxifying, activating blood circulation to remove blood stasis, invigorating spleen to resolve dampness, supplementing qi and nourishing yin,etc. [Conclusion] Professor Fan Yongsheng＇s experience in treating the sjojren syndrome is worth studying and popularizing in clinic.
