摘要
目的在低位直肠癌患者中施行Miles术（腹会阴联合直肠癌根治术），探究术中乙状结肠造口处使用IPOM法（即腹膜内固定法）预置BardCKTM疝补片对预防造口旁疝的作用。方法选取武警总医院肛肠外科于2015年2月～2015年6月期间行低位直肠癌Miles手术且经腹直肌乙状结肠造口患者25名，其中男性14例，女性11例，年龄45。69岁，平均56．2岁。在术中使用IPOM法将BardCKTM疝补片预置在乙状结肠造口腹膜处。结果所有患者术后均未出现补片或腹腔感染。25名患者，随访周期为14—18个月，平均16．8个月，均未发生造口旁疝。结论在低位直肠癌患者行Miles术时行IPOM法预置BardCKTM疝补片可预防造口旁疝的发生。
Objective To perform Miles＇ operation （Abdominoperineal Radical Operation of Rectal Cancer） on patients with low rectal cancer and investigate the effect of prophylactic Bard CKTM mesh placement through IPOM approach （Intraperitoneal Onlay Mesh Technique） during sigmoid-colostomy operation. Methods 25 patients were selected to undergone Miles＇ operation and sigmoidostomy through rectus abdominis muscle from February 2015 to June 2015 in our department of anorectal surgery,14 cases of men and women in 11 cases, aged 45 to 69, average age was 56.2 years. During the operation, Bard CKTM hernia mesh was used to fix into the sigmoidostomy peritoneal place through IPOM approach. Results All the patients did not have mesh or abdominal cavity infection after the operation. The average follow-up period was 16.8 months （range 14 to 18 months）.No case of parastomal hernia was found in the all 25 patients. Conclusion Performing prophylactic Bard CKTM mesh placement through IPOM approach during Miles＇ operation On patients with low rectal cancer can efficiently prevent parastomal hernia.
