摘要
目的分析某创伤专科医院住院患者医院感染现患率调查结果,找出感染防控重点,为有效预防控制医院感染提供科学依据。方法采用横断面调查的方法,分别对2012、2014和2016年某创伤专科医院住院患者医院感染现患率进行调查与分析。结果该院各年度医院感染现患率分别为7.08%、7.01%和6.74%;感染部位均以下呼吸道感染为主,分别占47.82%、45.38%和40.90%;每年检出的病原菌均以革兰阴性杆菌为主,主要为铜绿假单胞菌;重症医学科感染率最高;调查当日抗菌药物使用率分别为47.08%、40.24%和38.95%,预防性用药率为33.99%、32.64%和31.21%。结论该医院医院感染现患率呈逐年下降趋势,感染防控重点为呼吸系统感染,应有针对性加强对铜绿假单胞菌的防控,加强预防性应用抗菌药物的管理,对重点科室制定并采取防控措施。
Objective To investigate the prevalence rate of infectious diseases in trauma specialist hospital in China. Methods Adopt the method of cross-sectional survey trained medical workers consulted the medical records and relevant examination results of all in-patients who were hospitalized on the 3 days: May 16 2012, May 21 2014, and May 11 2016 0:00 -24:00 in the light of their clinical manifestations. The data were re-organized arid znalyzed by computer. Results The prevalence rates of infectious diseases on the specific survey days were 7.08%, 7.01%, and 6.74% respectively, with a tendency to gradually decrease. The infection site remained the lower respiratory tract on these 3 days (47.82%, 45.38% , and 40.90% respectively). The commonest pathogenic bacteria remained Gram-negative bacilli, mainly Pseudomonas aefuginosa. The highest infection rate was seen in the Department of Critical-care Medicine. The usage rates of antimicrobial agents on the very days of investigation were 47.08%, 40.24%, and 38.95% respectively, and the prophylactic drug use rates were 33.99%, 32.64%, and 31.21% respectively. Conclusion The infection rate in trauma specialist hospitas is decreasing gradually. The infection prevention and control should be focused on infection of lower respiratory tract and on Pseudomonas aeruginosa. Management of prophylectic drugs should be strengthened.
