摘要
目的探讨埃克替尼联合适形放疗及微波热疗治疗老年局部晚期非小细胞肺癌的疗效。方法选取2014年1月至2015年3月间山东省青岛市胶州中心医院收治的72例老年局部晚期非小细胞肺癌患者,采用随机数字表法分为观察组与对照组,每组36例。观察组患者行埃克替尼靶向治疗联合适形放疗及微波热疗,对照组患者行适形放疗联合化疗。观察两组患者的近期疗效、不良反应发生情况、疾病进展时间(TTP)及总生存期(OS)情况。结果观察组患者的总有效率和疾病控制率分别为83.3%和94.4%,均明显高于对照组患者的52.8%和80.6%,两组比较,差异均有统计学意义(均P<0.05)。两组患者总不良反应比较,差异无统计学意义(P>0.05)。观察组患者的中位TTP、中位OS和1年生存率分别为7个月、9.6个月和77.8%,明显高于对照组患者的6个月、8.1个月和52.8%,两组比较,差异均有统计学意义(均P<0.05)。结论埃克替尼靶向治疗联合适形放疗及微波热疗治疗老年局部晚期非小细胞肺癌患者具有较好的治疗效果,能够提高疾病控制率,延长总生存期。
Objective To explore the efficacy of icotinib combined with conformal radiotherapy and microwave hyperthermia for locally advanced non small cell lung cancer（ NSCLC） in elderly patients.Methods Select 72 elderly patients with advanced NSCLC who were treated in Jiaozhou Central Hospital from January 2014 to March 2015. According to the random number table method,they were randomly divided into an observation group and a control group,with 36 patients in each group. Patients in the observation group received icotinib targeted therapy combined with radiotherapy and microwave hyperthermia and patients in the control group received conformal radiotherapy. The overall efficacy,side effects,time to progression（ TTP） and overall survival（ OS） were observed. Results The overall efficacy rate and disease control rate was 83. 3% and 94. 4% respectively for the observation group,which were significantly higher than 52. 8% and 80. 6% of the control group（ P 0. 05）. There was no statistically significant difference in the overall adverse reactions between the two groups（ P 0. 05）. In the observation group,the median TTP,the median OS and the first year survival rate was 7 months,9. 6 months and 77. 8% respectively,which were significantly higher than 6 months,8. 1 months and 52. 8% months of the control group（ all P 0. 05）. Conclusion Icotinib targeted therapy combined with conformal radiotherapy and microwave hyperthermia has a good efficacy for locally small cell lung cancer in elderly patients,which can improve the disease control rate and prolong overall survival.
