摘要
目的:在上消化道隆起性病变的诊治中对超声内镜(EUS)的临床应用价值进行评价。方法:选自2011年3月~2016年8月在笔者所在医院接受诊治的上消化道隆起性病变患者556例,对所有患者行活组织病理学检查、EUS检查及电子胃镜检查,另外对其中344例患者进行CT检查,后根据患者病情施以内镜下切除或手术切除治疗。记录所有患者上消化道隆起性病变病理检查结果,比较EUS、胃镜和CT对所有患者的检查符合率,不同病种的EUS诊断超声声像学特点以及采取治疗方式治疗后的疗效。结果:经EUS和胃镜诊断符合率分别为93.9%和69.8%,EUS诊断符合率明显高于胃镜诊断符合率,差异有统计学意义(P<0.05)。结论:EUS诊断上消化道隆起性病变具有较高的准确率,对上消化道隆起性病变患者的内镜下治疗具有重要的指导意义。
Objective：To observe and explore the clinical value for diagnosis and therapy of submucosal protrusion lesion of upper gastrointestinal tract by endoscopic ultrasonography（EUS）.Methods：556 patients with submucosal protrusion lesion of upper gastrointestinal tract acceptedtreatment in our hospital from March 2011 to August 2016 were selected and given histopathologic examination and EUS examination,while 344 patients with CT examination.The treatment of endoscopic or surgical resection were given to different patients according to their condition.The histopathologic examination results,coincidence rate of EUS,gastroscope and CT examination,ultrasonic acoustic image characteristics of EUS and curative effectwere recorded and analyzed.Results：The consistent coincidence rate of EUS and gastroscopy were respectively93.9%and 69.8%,the EUS diagnosis coincidence rate was significantly higher than that of gastroscope,the difference is statistically significant（P〈0.05）.Condusion：EUS can diagnose submucosal protrusion lesion of upper gastrointestinal tract patient with high accuracy,and it has an important guiding significance for the treatment with endoscopic treatmen.
