摘要
目的观察益气泄浊和络方联合中药灌肠治疗CKD3~4期气虚湿瘀型患者临床疗效,并探讨其对肾小球滤过率(eGFR)、尿素氮(BUN)、血肌酐(Scr)、血清胱抑素C(Cys C)、血清钙(Ca^(2+))、血清磷(P^(3+))、血清碱性磷酸酶(ALP)、血红蛋白(HB)的影响,旨在延缓慢性肾脏病患者的病程进展,改善远期预后。方法将60例患者随机分为治疗组30例和对照组30例,治疗组在基础治疗上采用益气泄浊和络方联合中药灌肠治疗,对照组在基础治疗上加用肾衰宁治疗。疗程12周。比较两组治疗前后e GFR、BUN、Scr、Cys C、Ca^(2+)、P^(3+)、ALP、HB的变化。结果治疗组和对照组总有效率分别为86.6%和73.3%,组间比较差异有统计学意义(P<0.05);两组病例治疗后e GFR、BUN、Scr、Cys C、Ca^(2+)、P^(3+)、ALP、HB与本组治疗前比较差异有统计学意义(P<0.05);两组病例治疗后BUN、Scr、Cys C、Ca^(2+)、P^(3+)、ALP、HB比较差异有统计学差异(P<0.05)。结论益气泄浊和络方联合中药灌肠可早期干预肾脏代谢及内分泌功能,减少并发症,提高生活质量,延缓进入终末期肾脏的风险。
Objective To observe the clinical therapeutic effect on CKD3～ 4 of qi deficiency and damp stagnation treated with yiqi xiezhuo huoluo formula and Chinese herbal enema,explore the impacts on e GFR,BUN,Scr,Cys C,Ca^2＋,P^3＋,ALP and HB so as to retard the progress of chronic kidney disease（ CKD） and improve the long-term prognosis. Methods Sixty patients were randomized into a treatment group（ 30 cases） and a control group（ 30 cases）. Except the basic treatment,yiqi xiezhuo huoluo formula and Chinese herbal enema were used in the treatment and shen shuai ning was used in the control group. The duration of treatment was 12 weeks. The levels of e GFR,BUN,Scr,Cys C,Ca^2＋,P^3＋,ALP and HB were compared before and after treatment in the two groups. Results The total effective rate was 86. 6% in the treatment group and was 73. 3% in the control group,indicating the significant difference in comparison（ P〈0. 05）. The levels of e GFR,BUN,Scr,Cys C,Ca^2＋,P^3＋,ALP and HB after treatment were different significantly as compared with those before treatment within group（ P〈0. 05）. In comparison between the two groups,BUN,Scr,Cys C,Ca^2＋,P^3＋,ALP and HB were different significantly（ P〈0. 05） and the difference was not significant in e GFR（ P〉0. 05）. Conclusion The early intervention with yiqi xiezhuo huoluo formula and Chinese herbal enema benefits renal metabolism and endocrinal function,reduces complications,improves living quality and delays the progress to the terminal stage of CKD.
