摘要
目的研究以天癸散敷脐对围绝经期妇女的骨量减少进行干预,观察其临床疗效。方法101例骨量减少的围绝经期妇女随机分为两组,干预组51例,对照组50例,两组受试对象均给予基础治疗,干预组给予天癸散敷脐,疗程为3个月,对照组给予安慰剂(凡士林)敷脐,采用VAS疼痛评分评定疗效,并观察治疗前后骨密度(BMD)、骨源性碱性隣酸酶(BALP)、抗酒石酸酸性憐酸酶-5b(TRACP-5b)以及转化生长因子-β1(TGF-β1)等指标。结果干预组干预后能明显降低患者VAS评分和BALP和TRACP-5b含量(P<0.001),提高腰椎骨密度T值和TGF-β1水平(P<0.001)。两组干预后VAS评分、TRACP-5b和TGF-β1比较差异有统计学意义(P<0.01,P<0.05)。结论天葵散可以通过提高患者血TGF-β1含量,全面调节成骨细胞和破骨细胞的活性,从而起到提高患者腰椎T值和改善临床症状的作用。
Objective To study the intervention on osteopenia in perimenopausal women with tiangui powder on the umbilicus and its clinical therapeutic effect. Methods One hundred and one perimenopausal women of osteopenia were randomized into two groups,an intervention group（ 51 cases） and a control group（ 50 cases）. The basic treatment was applied in both of the two groups. In the intervention group,tiangui powder was used on the umbilicus. In the control group,the placebo（ Vaseline） was used on the umbilicus.VAS was adopted to assess the therapeutic effect. The duration of treatment was 3 months. Before and after treatment,BMD,BALP,TRACP-5b and TGF-β1were observed. Results In the intervention group,after intervention,VAS score and the levels of BALP and TRACP-5b were reduced apparently（ P〈0. 01）,T value of BMD in the lumbar vertebra and TGF-β1were increased（ P〈0. 001）. After intervention,the differences were significant in VAS score,TRACP-5b and TGF-β1between the two groups（ P〈0. 01,P〈0. 05）. Conclusion Tiangui powder regulates osteoblast and osteoclast activity through improving the level of TGF-β1so as to improve T value of lumbar vertebra and relieve the clinical symptoms in the patients.
