负压封闭引流技术在肢体严重创伤中的应用 被引量:4

摘要 目的:评价负压封闭引流(VSD)技术在肢体严重创伤中的临床应用价值。方法2013年1月至2015年12月对36例肢体严重创伤患者的创面予以负压封闭引流技术进行处理,其中合并主干动脉损伤6例,骨折18例,单纯大面积软组织碾压伤、挫裂伤或撕脱伤12例。结果除1例股动脉损伤合并股骨骨折需行截肢外,余35例经一次或多次 VSD 技术处理,创面最终经植皮或皮瓣修复,均成功保肢并创面愈合。随访1年后,35例植皮或皮瓣修复术后患者创面皮肤纹理好,功能理想。结论负压封闭引流技术在肢体严重创伤中可使创面得到有效的覆盖、引流和保护,为后期的植皮或皮瓣修复创造良好的条件,在保肢和覆盖创面的治疗中有重要的临床应用价值。 Objective To investigate the clinical efficacy of vacuum sealing drainage(VSD)on limb major trauma. Methods From January 2013 to December 2015,VSD combined with dermatoplasty was used to treat 36 patients with limb major trauma. Six cases were accompanied by major arterial inju-ries,18 cases were accompanied by fracture and 12 cases by mangled injury,avulsion injury or contu-sion. Results Following VSD treatment,granulation tissues grew well and dermatoplasty combined with VSD was used to treat and repair the wound surfaces of all cases except one case was amputated due to in-jury of femoral artery. The patients were followed up for 1 year and 35 cases with skin flap grafts sur-vived. The free skin graft texture on the wound surface was good,and the recovery of limb function was satisfactory. Conclusions VSD combined with dermatoplasty used for the treatment of limb major trauma may manage to provide favorable drainage,protection and show benefit for the wound healing,which is a simple and effective treatment method.
