股腘动脉闭塞腔内成形术远期预后分析

Long-term prognosis analysis of femoral popliteal artery occlusion angioplasty

摘要 目的:探讨股腘动脉闭塞腔内血管成形术对股腘动脉闭塞患者远期预后的影响,为临床治疗提供参考依据。方法回顾性分析2010年1月至2012年1月股腘动脉闭塞患者73例(81条肢体)的临床资料,按照不同术式分为两组,观察组42例(49条肢体)患者行腔内血管成形术治疗,对照组31例(32条肢体)患者行旁路转流术治疗。比较两组患者围术期指标及踝肱指数(ABI),随访12~36个月,观察两组一期、二期通畅率及保肢率。结果观察组手术时间、术中出血量、术后恢复行走时间均低于对照组,差异有统计学意义(P 〈0.05);术后两组患者 ABI 均显著降低(P 〈0.05),观察组与对照组比较差异未见统计学意义(P 〉0.05);观察组二期通畅率及保肢率分别为79.55%和84.09%,均高于对照组(P 〈0.05)。结论腔内血管成形术有助于促进股腘动脉闭塞腔患者早期康复,并显著改善了患者远期预后,值得临床重视。 Objective To investigate the effect of femoral popliteal artery occlusion angioplasty on long-term prognosis of femoropopliteal artery oclusion,and to provide reference for clinical treatment. Methods From January 2010 to January 2012,the clinical data of 73 patients(81 limbs)with femoral popliteal artery occlusion were retrospectively analyzed. The patients were divided into two groups accord-ing to the different surgical methods. The patients in observation group(42 cases,49 limbs)were treated by endovascular forming therapy and the patients in control group(31 cases,32 limbs)underwent bypass surgery. The perioperative indicators and ankle brachial index(ABI)of the two groups were compared. The patients were followed up for 12 to 36 months. The primary and secondary patency rates,limb sal-vage rates of the the two groups were observed. Results The operative time,blood loss,postoperative recovery of walking time of the observation group were lower than those of the control group,the differences were significant(P 〈 0. 05);the ABI of the two groups postoperative decreased significantly (P 〈 0. 05),but the difference between the two groups was not significant(P 〉 0. 05);the secondary patency rate and limb salvage rate of the observation group was 79. 55% and 84. 09% ,respectively, which were higher than those of the control group(P 〈 0. 05). Conclusions Endovascular angioplasty is helpful to promote the femoral popliteal artery occluded cavities in patients with early rehabilitation, and significantly improve the long-term prognosis of patients,it is worth clinical attention.
