摘要
目的:探讨帕罗西汀联合糖尿病健康教育对早期2型糖尿病患者糖代谢、内分泌和焦虑抑郁情绪、述情障碍的影响。方法:选择40例早期2型糖尿病患者,随机分为干预组和对照组,每组各20例。对照组患者接受糖尿病健康教育;干预组患者在对照组基础上联用帕罗西汀治疗。治疗24周后,分析对比治疗前后两组患者空腹及餐后2 h血糖、糖化血红蛋白、血清甲状腺激素及汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)、多伦多述情障碍20个条目量表评分变化。结果:干预组患者16例完成治疗。干预后,干预组患者餐后2h血糖(10.31mmol/L比9.11 mmol/L)、糖化血红蛋白(6.67%比5.97%)明显低于治疗前水平(均P<0.05),而血清甲状腺激素治疗前后未见明显变化。干预后,干预组患者与干预前比较,HAMA量表总分(10.50分比7.63分)、HAMD量表总分(11.81分比7.38分)、外向性思维评分(27.13分比22.19分)差异显著(均P<0.01);干预后,干预组患者与对照组的HAMD量表评分(11.25分比7.38分)比较差异明显(P<0.05)。结论:对早期2型糖尿病患者在糖尿病健康教育的基础上给予抗抑郁药帕罗西汀治疗,可改善其糖代谢及焦虑抑郁情绪。
Objective: To explore effects of Paroxetine combined with diabetes health education on glucose metabolism,endocrine,anxiety-depression emotions,and alexithymia in patients with early type 2 diabetes mellitus. Methods: 40 patients with early type 2 diabetes mellitus were recruited and randomly divided into Paroxetine group( intervention group) of 20 cases and control group of 20 cases. All patients received diabetes health education,while the patients in Paroxetine group were treated with Paroxetine,additionally. The fasting blood glucose,2h postprandial blood glucose,glycosylated hemoglobin A1 c,serum thyroid hormones levels as well as the scores of Hamilton anxiety scale( HAMA),Hamilton depression scale( HAMD),and Toronto Alexithymia Scale- 20( TAS-20) of the two groups before and 24 weeks after the treatment were analyzed and compared. Results: 16 cases of intervention group had completed the treatment. After the treatment,the 2h postprandial plasma glucose( 10.31 mmo L / L vs 9.11 mmo L / L) and glycosylated hemoglobin A1c( 6.67% vs 5.97%) in intervention group were decreased significantly than the baseline values( all P〈0.05); however,the levels of serum thyroid hormones had no differences with those before the treatment. The differences of the HAMA total score( 10.50 vs 7.63),the HAMD total score( 11.81 vs 7.38),and the externally oriented thinking score( 27.13 vs 22.19) in intervention group before and after the treatment were significantly( all P〈0.01). There was a significant difference in the HAMD total score( 11.25 vs 7.38) between the intervention group and control group( P〈0.05). Conclusions: Paroxetine combined withdiabetes health education for THE patients with early type 2 diabetes mellitus may probably be helpful in improving the patients' glucose metabolism and anxiety-depression emotions.
