摘要
目的比较不同来源单个核细胞治疗2型糖尿病(T2DM)下肢血管病变的有效性及安全性。方法选取河北医科大学第一医院内分泌科T2DM下肢血管病变患者60例,按随机数字表随机分为对照组(n=20)、骨髓组(n=20)及脐带血组(n=20),分别给予生理盐水、自体骨髓单个核细胞、脐带血单个核细胞小腿肌间注射。分别于第0、12、24周评估其有效性及安全性。结果治疗后,对照组各项指标差异无统计学意义(P>0.05);12周时,骨髓组静息痛评分、肢体冷感评分、麻木评分分别为4.56±1.04,3.67±0.84,2.72±1.07,F=15.093,20.292,31.463,P均<0.001与脐带血组静息痛评分、肢体冷感评分、麻木评分分别为4.89±0.83,3.44±0.98,2.50±0.79,F=26.792,12.967,35.034,P均<0.001,差异具有统计学意义;24周时,骨髓组间歇跛行距离、经皮氧分压、下肢皮肤的温度、静息状态下踝肱压指数分别为(310±65.5)m,(34.89±6.65)mm Hg,(32.29±1.33)℃,0.59±0.11,F=23.938,42.243,22.736,19.971,P均<0.001与脐带血组间歇跛行距离、经皮氧分压、下肢皮肤的温度、静息状态下踝肱压指数分别为(318±64.9)m,(31.78±5.63)mm Hg,(32.83±1.02)℃0.61±0.07,F=25.726,13.542,38.113,33.510,P均<0.001,差异具有统计学意义,同时间点两组间差异无统计学意义(P>0.05),研究过程均无死亡、肿瘤、增殖性视网膜病变病例报告。结论两种来源单个核细胞小腿肌间注射治疗T2DM下肢血管病变均安全、有效,脐带血单个核细胞的疗效不劣于自体骨髓单个核细胞。
Objective To compare the efficacy and safety of injection of different mononuelear cells for lower-extremity arterial disease (LEAD) of type 2 diabetes (T2DM). Methods 60 patients with LEAD of T2DM were randomized into control group (n = 20), umbilical cord blood group (UCB group, n = 20) and autologous bone marrow group (ABM group, n = 20), wwhile normal saline, umbilical cord blood stem cell and autologous bone marrow mononuclear cells were respectively injected between the calf muscles of them. The subjective and objective responses were measured at week 0, 12 and 24, while safety was evaluated as well. Results There were no significantly change of the subjective and objective outcomes in the control group during the study (P 〉 0.05). At week 12, scores of rest pain, cold feeling and numbness were 4.56± 1.04 (F = 15.093, P 〈 0.001 vs control), 3.67±0.84 (F = 20.292, P 〈 0.001 vs control) and 2.72± 1.07 (F = 31.463, P 〈 0.001 vs control) respectively in the ABM group, and 4.89±0.83 (F = 26.792, P 〈 0.001 vs control), 3.44±0.98 (F = 12.967, P 〈 0.001 vs control)and 2.50±0.79 (F = 35.034, P 〈 0.001 vs control) respectively in the UCB group, which were significantly improved compared to the control. At week 24, intermittent limp distance, transcutaneous oxygen pressure, skin temperature of lower limb and ankle brachial pressure indices were (310± 65.5 ) m (F = 23.938, P 〈 0.001 vs control),(34.89±6.65)mmHg(F = 42.243, P 〈 0.001 vs control) ,(32.29±1.33) ℃ (F = 22.736, P 〈 0.001 vs control) and 0.59±0.11 (F = 19.971, P 〈 0.001 vs control) respectively in the ABM group, and (318 ±64.9) m (F = 25.726, P 〈 0.001 vs control), (31.78± 5.63 ) mmHg (F = 13.542, P 〈 0.001 vs control),(32.83 ± 1.02)℃(F = 38.113, P 〈 0.001 vs control)and 0.61±0.07 (F = 33.510, P 〈 0.001 vs control) respectively in UCB group, which were significantly improved too.
